Below looks OK for someone ON thyroid medication. She was still on t4 and t3, correct?
It very much resembles someone on both meds because the TSH is slightly low in range while the frees are near middle range, not high.
TSH 1.690 (.450-4.5)
T3 Free 3.0 (2-4.4)
T4 Free .98 (.82-1.77)
Reverse T3 17.5 (9.2-24.1) .....this improved, I think it was 24 once?
T3 104 (71-180) ......outdated test but still looks good.

As a few of us here discussed last week, this lab has weird ranges for antibodies. Perhaps they use older equipment. There is usually a more accurate single digit cut-off single numbered value for these, not a range.
So with this "range", looks like all is well but the Ta as you also noted. Its high, but not very much. Technically that is the early stage of Hashimoto. These can also fluctuate in the beginning. I would retest that one in a 3-6 months maybe.
