I'm so sorry you're dealing with this, but a lot of it sounds familiar! MCAS can be so challenging.
Binders in general cause constipation, so if you need to avoid that (and really everyone should avoid that) than I'd advocate using good old vegetables as your binder. Dr. Rawls says that for every meal, half your plate should be vegetables, and Neil Nathan has a whole chart of which foods act as binders for various mycotoxins. So whatever plant matter you tolerate, dig in.
For stabilizing mast cells, are you able to avoid all other mast cell triggers (or many of them) when you are taking antimicrobials? Your body may be able to react less if it's only dealing with endotoxins from dead bacteria, rather than other mast cell triggers like stress, exertion, cold, heat, sensory stimulation (like showers, noise, etc.), allergens, mold, high-histamine foods, high-histamine supplements, etc.
Mast cell stabilizers to try: DAO (porcine or legume-derived), ketotifen, cromolyn sodium, H1 blocker antihistamines (e.g. Claritin), H2 blocker antihistamines (e.g. Pepsid), bromelain, curcumin, quercetin, stinging nettles, L-theanine, khella, perilla, pine bark, luteolin, rutin, mangosteen, evening primrose, quercetin, vitamin c, ECGC/green tea, milk thistle, butterbur, feverfew, baikal skullcap, DGL (form of licorice), montelukast, chamomile, moringa, NAC, cannabis (full-spectrum or CBD-only), pomegranate, mango, limbic system retraining programs (DNRS, Gupta, etc.).
To support your body's own DAO production: onions, green tea, galangal (Thai ginger), peaches, Brazil nuts, fiber, sprouted lentils, sprouted chickpeas, olive oil, walnuts, omega 3s, saturated fat, avocado, almonds, white beans, leafy greens, legumes, cashews, pumpkin seeds, iron, zinc, phosphorous, B12, magnesium.
There's a strong relationship between uterine symptoms and histamine, probably accounting for your heavy bleeding and spinal pain. This can be exacerbated by other situations, like endometriosis and perimenopause, partly because of the relationship between histamine and estrogen, and partly because the uterus is closely tied to the immune system. Some even argue that endometriosis is an autoimmune condition. I have made a lot of progress since discovering I had MCAS, but I still have a ridiculously dramatic reproductive system, so I'll be sure to let you know if I ever figure that out! My doctors are fond of vitex, magnesium, iron, zinc, and managing histamine. HPA axis dysfunction (hormonal imbalances in the hypothalamic-pituitary-adrenal axis) requires strict attention to the basics (sleep, movement, stress, nutrition), eschewing stimulants (coffee, sugar), and possibly trying adaptogenic herbs, vitex, or supplemental hormones if necessary.
Mainstream medicine will tell you that literally the only tool in their toolbox for this is synthetic hormones, which of course carry their own risks and side effects. One gynecologist suggested I take lupron to induce menopause and then add in balanced amounts of synthetic estrogen and progesterone. A friend finds it helpful to take continuous hormonal contraceptives, never allowing her body to menstruate or have hormonal fluctuations. But of course those are only good options if you tolerate synthetic hormones well.
I wish I could tell you exactly how to fix this!

Wow so much great information! I have to give it more time as I just dealt with major die off and heavy bleeding. Just got the bleeding to slow down using some acupuncture. But what helped most was I pulled out some activated charcoal i never
opened and bottle is probably a decade old and took a pill. I never thought I would see slowing down of my bleeding without my usual interventions. Albeit this took several hours and several doses. I used black seed oil and i didn't realize the strong eradicating effect it would have on my gut microbes. I believe I have a lot of endotoxins. Having lyme and going through many different medication withdrawals have thrown my body out of whack years ago and it was a fruitful environment for bugs to thrive in me. Which is a life or death situation when I try to eradicate anything. How I know these bleeding occurrences are about
to happen is after I use an anti-microbial, I'll start to experience the burning pain up along my spinal column up to the left of my neck. I can barely hold my back up. Realized this is the massive release of mast cells/toxins from die off.
I don't know how to incorporate black seed oil without causing this massive reaction. I tried fruits and veggies and i'm long past that point. I need to bring in big guns. I don't mind it being constipating as I can use a natural laxative such as magnesium oxide. But I'm more concerned about
it disturbing my good bacteria. Im only going to use Activated Charcoal for 2 days? Dont want it to absorb everything. I'm sure i'm at a deficit in regards to any nutrients or good bacteria. I've heard of bentonite clay to bind toxins out of body. Not sure how it's different from Activated charcoal. Also heard many good things about
Zeolite pure but not sure if there are any warnings about
using these. other then the constipating after effect. Any insight on these?
