My understanding is that most people on IV abx get them via PICC line and do the abx infusions themselves at home (after being properly trained).
There is always a chance of infection with PICC lines. However, if you pay very careful attention to proper PICC line access procedures, you can significantly reduce the chances of infection. I've had my PICC for 4 mos. with no hint of infection. I am aggressive with cleanliness - always washing hands 20 seconds with antibacterial soap plus using hand sanitizer (if wearing gloves, alcohol swab the gloves before touching the PICC line), using tons of alcohol swabs (when it doubt, swab twice!) to keep the endcaps clean, never allowing just anyone to access the line without a very good reason and/or proper training, always making sure my work surface (for setting supplies on, etc.) has been cleaned thoroughly with anti-infective cleaner, etc.
