Hassle with UHC insurance

My insurance (UHC), is hassling me about my bloodwork I had done, which included my 2nd CD-57.  They are requesting the Progress notes, test results, and clinical records from my doctor.  The first time I had my CD-57 done they didn't pay for it, calling it experimental, now this.  I think they have an alarm that goes off whenever someone has a CD-57 done that says, hey someone else thinks they have "Chronic Lyme" time to hassle them.  Like we need any more headaches!  What a PIA!

I had UHC briefly through my work, and they were terrible. I paid them $1000's in premiums, and they paid next to nothing of my medical bills...and then they went and messed up their customer service phone system and I could never get in touch with them to ask questions after that...good thing I had other much better insurance at the time...
