The "Mild chronic small vessel ischemic changes" can also be attributed to Lyme disease:

The issues you mentioned - forgetting how to tie shoelaces, turning off the stove and so on, may be from ammonia in the brain or from the infection progressing. I know for me, I had serious issues with things like that both before I started treatments and then when I was herxing badly.
