Delmar,
You took a good step looking for a llmd. It can sometimes take a while to get an appointment.
You have indeed a lot of symptoms that fit with Lyme Disease, and you live in a rural area.
Lyme testing is very inaccurate, a good llmd should be able to diagnose you without a positive test, based on his clinical findings.
What you could do is start a diet: go off sugar and dairy, try to eat as less gluten or gluten free if possible, and eat as healthy as possible.
You can inform yourself about detoxing methods and eventually start them (Have you already gone through the thread on top of the Lyme page 'New to Lyme? ... Start here!'. There is a lot of useful information there). It will be easier to adapt to lyme treatment, if you got those daily stuff already planned into your daily life. It can be quite overwhelming if you have to start treatment, diet and detoxing all at the same time, especially when you're herxing.
Don't hesitate to post if you have any questions!
Keep us posted on how you're doing and on possible appointments,
MarieLS
