Totally depends on whether or not you're going to see an LLMD.
If it's an LLMD, they're most interested in the list of signs and symptoms you have, and the history of the illness. If you had a tick bite and a rash, a date would help.

When I met with my doc the first time, I told it as a story. We met for nearly two hours, which included the physical exam and planning of treatment. So it helps to have notes, organizing your symptoms either by chronology or by body system.

The doc will also be keenly interested in what you DON'T have, so all test results are important in narrowing down the diagnosis.

I will have my first appointment with an LLMD at the end of December, they have sent me about 6 pages of papers they wanted to fill out. Most of it is about the symptoms onset duration etc... They also asked me to bring any medical records etc, that relate to my condition. I can't wait to see my new doctor even though I have to pay out of pocket.
None of my family doctors or the ones covered by my insurance took me serious. I always had the feeling as soon as you mention the word lyme they don't want to have anything to do with me. They never bothered trying to help me
