Totally depends on whether or not you're going to see an LLMD.
If it's an LLMD, they're most interested in the list of signs and symptoms you have, and the history of the illness. If you had a tick bite and a rash, a date would help.
When I met with my doc the first time, I told it as a story. We met for nearly two hours, which included the physical exam and planning of treatment. So it helps to have notes, organizing your symptoms either by chronology or by body system.
The doc will also be keenly interested in what you DON'T have, so all test results are important in narrowing down the diagnosis.
