Yup, I know those pains well. Well, it is a good thing that your blood work and ECG are normal. You may want to get some Gaviscon liquid and when you feel like this, take a sip of it and follow it with some water.
Gaviscon will quickly turn into foam and will cover the walls of the esophagus and the stomach as well as neutralize any acid that is causing this. If it is reflux or acid related you will feel immediate relief. It may only last a short while but at least it answers your question and it takes some of the worry away.
If you find that it is reflux or GI tract related, you may want to try DGL licorice tablets. They help heal ulcers and coats the lining of the stomach so that eventually the symptom is not so noticeable.
If you have not been tested for H. Pylori or other stomach issues, then you might want to look into it.
As to the pain being different, the stomach is towards the middle left side of your chest while the esophagus pains seem to radiate to the middle right. If you are having gallbladder issues, the pain will also radiate towards the right.
Then there is the vagus nerve. this nerve is linked to the GI tract, the heart, your esophagus, your diaphragm, your colon, uterus, bladder and so much more. When this nerve is impacted by Lyme and Co then you end up with so many weird pains and symptoms in the aforementioned organs. These pains come and go just like how you are experiencing them.
I should tell you that if possible, you should get an ultrasound of your major arteries. My local hospital does it for about $70 dollars and you don't need insurance. My hospital does an ultrasound of the heart, main aortas, the neck, legs, arms, and chest. If there is any calcification of the arteries or obstruction in the blood flow it may cause some of the symptoms that you mentioned. Play it safe and make sure this is not the issue, it can be fatal if it is.
