Typhoid is very very rare in NZ and a growing 'epidemic' in Fiji, with 24 reported cases this past New Year's in a population of just under a million. They believe it spread among the guests at a NYEve Party.
You can avoid typhoid fever by washing your hands well, drinking purified water, and eating only well-cooked food while on your fabulous vacation :). Harder to avoid is Dengue Fever in Fiji, which is carried by mosquitoes. Weigh this info against the possibility of exacerbating your lyme symptoms by getting a vaccine or two...
