I have finally found a local doctor that treats lyme but unfortunately she seems more focused on IV therapy than oral. I let her know that I much prefer to treat with oral antibiotics and she said that she would be glad to work with me. A couple of weeks ago I began taking Cipro and Zyvox(linezolid). I completed the zyvox prescription and I asked to discontinue the Cipro out of concerns for my tendons. The follow up prescription which I am taking now is Keflex(cefalexin).

I've noticed that the treatment protocols utilized by many of the forum members and discussed in the treatment guidelines by Dr. B and the ILADS don't typically utilize the abx I have been prescribed. In fact they recommend against them..ie cefalexin..
Up until now I have been reluctant to request a specific antibiotic out of respect for the doctors expertise but I am ready to be a little more proactive.

I know there is not a "one size fits all" and I also know that it is uncomfortable to "prescribe" a protocol on the forum, but I am looking for "generally used" antibiotic protocols. I also realize that often drugs are paired such as Amox and Biaxin so knowing a general order of protocol and paired drugs would give me the basic knowledge to do more research before my appointment tomorrow.

Not to answer my own question.... But here is a list abx that are mentioned most often in literature and on this site.

Amox.. Doxy... Minocycline.... Rifampin...Biaxin... Tindazole

I'm sure I am missing many.

I have had a reaction (herx?) to amox months before while being treated for a resp. infection which leads me to think it may be a candidate. But why is it paired with Biaxin ?

Well, I can tell you what I've been on...that way I'm not prescribing.

Doxy paired with zithro

Amoxicillin paired with biaxin

Ceftin paired with biaxin

Pulsing Tinidazole two days a week - paired with Ceftin and biaxin

Minocycline paired with Rifampin (for bartonella treatment)

For a few dollars per month, you can subscribe to Dr. M.R.'s Lyme Treatment 'book' (online) and he has a lot of different protocols, as well as info on supporting the body functions while treating.

Why don't you print out the ILADS guidelines and show them to your doctor?

Thank you Girlie... That is very helpful

Sarah..that is a worthwhile idea ..at least to show her that I have a source for my request. Her office is very busy and i have found that researching my script after I have left the office leaves me a bit confused and disappointed. I have been reluctant to suggest a protocol up until now, after all, she is the doctor. Maybe the IV protocol she uses takes an entirely different approach to treatment and my request for oral antibiotics is unusual.

B777 - I understand how you feel about suggesting to the Dr. what protocol you want. When I wanted to go on minocycline (instead of doxy- because it's summer time and i reacted to doxy last year)...with the rifampin...I thought carefully about the 'approach' and decided it was best that I suggest it before he brought up doxy...and that way - I wasn't negating his decision.
It worked. I actually had prepared my 'script' prior to the phone appt.

It went something like this: "Since I was already on doxy last summer...and was super sun-sensitive while on it...I was thinking - do you think I could try minocycline? I've also read that it penetrated the blood-brain barrier quite well...and we have previously discussed I have a lot of CNS symptoms...it would go well with Rifampin...when we start that for bartonella treatment? What do you think?"

Then I crossed my fingers...and he said back to me, "yes, I think that would be a good choice."

"After all, she is the doctor". Unfortunately, I used to think like that too. I'd still be very sick if I didn't take matters into my own hands and start advocating for my own health. Doctors are not trained about these diseases in medical school and they are at the mercy of insurance companies. If you don't feel comfortable on a protocol, don't be afraid to speak up because you are the patient, it is your health, and you are the one paying for the doctor's help.

Yeah, B777 that was my intent. So you can show you are asking for a reason and there is a medical source, not just because you think it sounds good. Especially since ILADS specifically recommends against the medicine you were prescribed.
