The current thinking with IV Rocephin is to give it on a Mon, Fri, Wed schedule. That's based off a study and I did that for around 12 weeks. All of us doing that were also taking Ursadol the night before and the day of and after the IV to protect our gallbladder. I got great relief from my Lyme symptoms but in the later weeks found my Bart symptoms came barreling back.
Tick check I'm pretty sure Dr H is also using that protocol.

Never heard of pulsing the Rocephin. Interesting.

If I don't want to do a PICC, I wonder if I could pulse intramuscular injections.

Dr H might just want to do orals. I know they're powerful.

So if pairing with a Macrolide is for early Bart what's the pair for late Bart?

there's no difference in how you treat early or late stage bart it all depends on what abx combination works for you. i've read plenty of places where they say doxy, zithro, biaxin, etc. will work but i haven't seen it as a front line medicine work for anyone.

the tier 1's are rifamycin (rifampin, rifabutin) and fluoroquinolones (levaquin, cipro) also bactrim to a lessor extent. once again mileage may vary what works on one person may not work as well on another. the more important abx seems to be the first one in the prior list the second "supporting" abx is varied.

Rifampin is the 'go-to' antibiotic for Bartonella treatment.
It is paired with several different abx - doxy, mino, biaxin, azithromycin, bactrim...are some I have heard of.

Some LLMD's are now prescribing Rifabutin now...which is in the same class as Rifampin, but it is very expensive.

Rifampin can also be used for a bart infection that is 'newer'.

...and I know people who caught their bart early on and tried pairing macrolide with tetracycline and also tried BactrimDS , but didn't put their Bart into remission...so have switched to Rifampin.


*** The class of Fluoroquinolones are perhaps the most effective drug to treat Bartonella, but because of the black box warning - many LLMD's aren't prescribing it unless it's a last resort.

(risk of tendon rupture, and more recently peripheral neuropathy)

Thanks all. Yes I remember reading that tier structure.

I would like to avoid the Cipros etc if I can. I seem to tolerate Rifampin fine. Just need to find the right pair and do it for a lot longer.

Some bartonella species respond to cephalsporins ...dr b feels it puts it on pause ..it has helped with zith and venom and herbs and silver .gfields had some experience with im roceph ..i got the powder and the procaine just never got the hootspa...it can be had at 6$a gram from adc ..needles are the thing ..half life is shorter for im ...and 1 gram vs 2 is like 12 hr difference in mic
