Usually a clinic who hasn't seen you before will want copies of recent/pertinent medical records.
You don't have to book the appointment one week out...you could have chosen 2...or 3...or more weeks out so it wasn't such a rush to get all the documentation in order.
I agree with the forms to fill out - you end up filling out some of the same forms when you get there.
When I called, they told me they had an appointment next week. So I booked it. It was only after that they asked for all that paperwork. And they sent me an email saying to please complete the paperwork 7 days prior to your appointment. Well, that didn't happen! And then they called me and said, hey we need you to fill out those forms. I did get it done before the appointment.
I wish someone had explained all this to me beforehand.

Do you mean a NP? And didn't you choose one when you made the appt? I had an assistant do some vitals - ie blood pressure , pulse, weight...
but the NP was with me for 2 1/2 hours.....checking reflexes, eye-tracking, strength, and asking questions about certain symptoms.
NP or PA, I guess?
I just made the appointment, and then they gave me the name of who I would be seeing. I didn't know I could choose one. I wouldn't have known who to ask for.
Yes they did all that stuff. Eye tracking, stand on one foot, reflexes.

the MRI, I don't know. I guess I need to call and ask for clarification. I've also found that if you call after hours you get a recording that says call back during normal hours, and you can't even leave a message. When you call during business hours then it goes to voice mail.
