Hi, while on its own an indeterminate 31 and 41 wouldn't be huge alarm bells, their potential import is increased given your constellation of neuro symptoms imo. The clinical picture adds weight to those results (and it's really unfortunate we have to rely on this flawed 1950s testing tech to read the tea leaves), but that's the way the testing paradigm currently is. 31 is specific for borrelia, which I think may be a good clue coupled with the symptoms. It's not slam-dunk, but merits strong consideration.
Could you revisit the antimicrobial challenge? I've always liked that tool as an indirect method of getting more information. I understand not wanting to poke the bear. But if you take antimicrobials at strong enough dosage, and in fact engender a flare, at least you give yourself a bit more indirect confirmation that you are dealing with an underlying infection(s). Then you can always improvise the treatments to a level that is manageable.
