Try doing a Google Maps search for "Louisville, KY" and click the "Nearby" button. Search nearby for functional medicine, holistic medicine, etc.* Several practices pop up, some of which might treat Lyme but won't advertise it on the website, so you'll want to call and ask if they treat tick-borne disease. One practice that looks promising is Kentuckiana Integrative Medicine.

*I did not suggest searching for Lyme specifically because some infectious disease specialists appear, and many of them adhere to the ineffective and outdated IDSA guidelines with regard to Lyme treatment. Some infectious disease doctors are great, but a lot of us have had terrible experiences with them, so personally I'd opt for functional or integrative folks unless you have heard a specific word-of-mouth recommendation for an ID doctor.

Another option is to contact ILADS for physician recommendations.
