I am also a little surprised by the results for someone taking only T4 - would have expected high normal T4 and low T3
on the face of it you are pretty well medicated - and the free T3 is more important than the free T4 levels - as the T3 is the main biologically active form and T4 is in any case still in range.
i have been in the category of high normal range T3 and low normal range or just under T4 - and felt about
as good as i ever have on that ( but this was from combination T3 / T4 medication ) i have since added more T4 and less T3 to try to get T4 into optimum ( upper half of range ) - but have not really noticed any benefit from that change.
i have also had odd lab test results - where out of the blue after a series of very stable results for maybe 6 in a row - and no changes to medication - i get one that looks like a wild card - there could be two reasons for this
1 - the lab simply had a rogue result due to some process or reagent error - my dr says this does indeed happen believes this to be the case in my recent TSH at 0.08 result
2 - the thyroid condition itself - especially if Auto-immune driven ( Hashimoto's etc ) is a fluctuating condition with periodic flares and remittances in many people. when Thyroid tissue is attacked and broken down - thyroid hormones are released and blood levels can increase. It is very likely this is not a steady-state condition in most patients - and all have some fluctuation to some degree whether they are aware or not.
so it could be you are under medicated most of the time - and showing hypo symptoms like weight gain - but with a small temporary flare in blood levels due to auto-immune action.
i think if it were me I would not change anything based on this result - but instead keep everything the same and test again in a few weeks and see where you are. especially if you still have other hypo symptoms - hair thinning, cold extremities, low energy ( oh, wait, who doesn't have low energy with Lyme ....)
