I did a phage testing from RED laboratories and it came back positive for Borrelia Miyamotoi. I am not familiar with this type. Is this the cause of all my symptoms?

I have no familiarity with that specific form.
It’s possible to have one infection cause symptoms, particularly in short term/acute situations.
If you consider yourself more in the chronic illness realm and this has been going on for sometime.. I at this point see chronic illness as a breakdown on several levels. Bacterial, viral, parasitic, fungal, toxins etc.. with that said some people get lucky and check one microbe and the body handles the rest but assume it was just Lyme or whatever. if I could go back I would treat comprehensively and not bother testing as there’s more viruses in us than we currently have the ability to identify (healthy people too).

Myamotoi is a strain that (according to llmd Cameron) came to the US in 2013

red labs phage based testing is interesting - but be aware that it has not been validated or replicated by any 3rd parties so we have little data with which to interpret its specificity or sensitivity - the same could be said of other labs of course - but particular caution is merited here because it is one of the smaller independent labs, is less used labs by LLMD's so we have less skilled clinicians forming a view from parallel testing of their patients, and it uses a technology no one else is using.
myamotoi is one of the relapsing fever group spirochetes - symptom patterns seem highly variable - several members here have tested positive through other labs - and do not necessarily report relapsing fevers.
as mentioned by Loski - part of the variability of symptom patterns may be due to the common presence of co-infections - and /or immune disfunction.
in general it is treated with the same anti-biotics as Lyme disease - but this is empirically based as there are few studies that demonstrate efficacy of different abx - one paper published says it is susceptible to doxycycline, azithromycin, and ceftriaxone but resistant to amoxicillin in vitro. no comparative trials in patients as far as i know.
