LLMD near Chicago?

I am in the north suburbs of Chicago. I am 2 years into my 2nd Lyme infection. The first was acute and treated within a week about 13 years ago although I have had some mild persistent symptoms, but nothing that was worth worrying about.

This 2 infection I got about 2 years ago didn't come on nearly as strong as the first (104 degree fever and bedridden with joint/muscle pain within a couple days). So it took nearly a year to diagnose.

I am desperate to find a recommendation for an LLMD in Chicago, WI or IN or perhaps virtual. I would even be willing to go virtual or travel if it would help.
