Enhanced Brain Lesions

It seems that only new brain lesions would be enhanced. The old ones should look old and dried up and the new ones should be bright and lively. What is the real story about those?

And how do very old lesions that are not enhanced still cause trouble? Why were they quiet for so long and start causing problems now?

I think you have the real story. New active lesions show up on an MRI with contrast as white. Old lesions show up at gray or black. Old lesions can cause symptoms.

In MS, when there is a flare, old lesions can activate again and cause worsening symptoms. During a flare, a new lesion can develop. This would cause new symptoms.
