Jpouch Bleeding... One month after takedown surgery.

Good Evening All,

For a start, I hope I am posting in the correct forum!

Just a quick summary of my history with my IBD... I was diagnosed with Ulcerative Colitis in 2005 (at the age of 15), symptoms became unbearable and I was operated on as an emergency in 2007 where my end ileostomy was born!

I suffered with adhesions and twisting of the small bowel a short while after and was operated on again in 2008... From this I kept my end ileostomy for nearly 7 years as I completed all my studies, achieved a degree and got comfortable in a job :-) January this year, I was operated on to have a Jpouch constructed. Then in July I had the takedown surgery to join everything back together. I am now 23!

I have had an eventful 4 weeks with the new pouch, but have a concern or 2 which I am hoping I can share experiences with and rest my worries - and hopefully learn a thing or two!

Every now and again (once every couple of days maybe) I notice streaks of blood in my stools (usually in 1-2 BM's in the evening time). It isn't a great deal, maybe sometimes there is evidence on the toilet paper. I have no other major symptoms, sometimes some 'butt burn' and anal canal itchiness...

I now empty my pouch between 6-10 times a day and maybe 1-2 times at night (depending on what I've eaten, drank and what time I go to bed). I feel great in myself and can't wait to start getting back to work, gym, swimming, etc... Its just this once minor concern.

Has anybody had any other similar experiences they can share or any advice?

Hi Liam, I am about 4 weeks post takedown also :) It sounds like the blood might be coming from skin irritation or perhaps hemorrhoids? The evening is usually my "busiest" time of day so that might be when the irritation is worst and there is most likely to be bleeding. Just a guess. Bleeding can also be a sign of cuffitis (inflammation in the rectal cuff) but that *usually* presents with other symptoms. I think I would check in with your GI or surgeon and see what they want to do, but I really wouldn't worry too much. Sounds like you are doing great!

Most likely it's normal. According to my surgeon, the blood vessels lie close to the skin and can easily be irritated. I'm 12 years out and see blood on occasion. You should probably address this with your surgeon or GI, just to be sure. But if you are asymptomatic, most likely this it's not anything to worry about.

Liz, I had my takedown on the same day as you! :)
No evidence of bleeding since the occasion for me to start this thread - everything is settling down nicely. I was just concerned and wanted to know if the blood was normal, but I will mention in the outpatients clinic soon :)

How are you guys managing?
My pouch is also very "busy" in the evening... On a good day, I'll have 6-8 toilet trips in the day and I'll have 1-2 visits through the night.

I nearly have my full appetite back, so I am expecting this to change with the extra food.

I am doing very well, knock wood. Much better than I ever expected. I have 4-5 trips during the day and one in the middle of the night. I was able to start going for short jogs this week which has made me so happy -- I was able to jog after Step 1, but after Step 2 I never had any energy (my transit was very fast with the loop ileo, even with Imodium) so it was all I could do to get through the workday. I feel like I'm finally getting the nutrients from my food and that has made a big difference. I still have some anal irritation (manageable with bidet and applying A&D after every BM) and it can take me 10-15 minutes to fully empty, but otherwise I'm happy!
