End elio

An end ileo is an ileostomy where the very end of the small intestine protrudes as the stoma; this is in contrast to the "loop" ileo used to allow a jpouch to heal before the jpouch is connected to the fecal stream, and where the stoma is formed with a cut forming a twofer-- two protrusions, one for the upsstream small gut fecal stream and the other for mucus spilling back from the gut still healing in the newly constructed jpouch. This is my off the cuff definition, hopefully skmeone like pluot will chime in with a more medically precise definition. Step 1 of a 3 step jpouch as well as the single step of a single permanent ileo (or the permanent ileo after step 1 of 3 if one decides not to proceed on with a jpouch as was the case for me) form an end ileo. A 2 step jpouch starts off with a loop ileo before the ileo disappears altogether in step 2. And end ileo can and should be made so that it sticks out enough to never end up flush to the skin, so that stool plops out nicely into the pouch and it functions wonderfully; a loop ileo by its nature tends to be very flush with the skin and thus, for many, very problematic.

@Watermaster-- you don't use a barrier ring? I use an eakin on my hollister wafer and it works like a charm. I get 4-5 solid days out of it before changing-- sometimes 6, but I nirmally dont push it past 5 because I am very active and sweat and shower a lot. I thought that convex wafers are needed only for very flush stomas-- if yours is 2 inches it's hard to imagine that going flush. Mine varies from 1-1/4 inches to occasionally as little as 1/2-3/4" long. Anyway, sounds like you have found something that works-- that's all that matters.

@WaterMaster~ My stoma sounds similar to yours. A lot of the time it protrudes 1 1/2 inches or more, but after I had it for a while, I notice it started retracting until it was pretty much flush. This usually happens when I am laying down, but has happened a few times other than that. I use a Convatec wafer that is not available in convex, but they do have a convex insert. I use a barrier ring and the convex insert and that greatly helped cut down on the leaks I started to experience from the stoma shrinkage. I DO still have a leak from time to time, but usually I can feel when one is coming and change early if need be. Good luck!
