J-pouches are a physiologically bad idea

NCOT: My first surgeon was a horrible woman. Her idiot staff put one of those old solid wafers over my incision, which caused it to open in the middle when I went to get the staples out. The surgeon blamed ME and my mother for letting it happen. No one ever gave us after-care instructions, I was twelve years old and still very surprised by everything that was happening to me. My later doctors were much better thankfully. Hell, the loop ostomy my second surgeon put in lasted me fifteen years before the stoma began to retract. I'd call that good work. My current surgeon is good, too. He makes a very clean incision and must have scooped out the old scar tissue that caused my tummy to look like the top of a loaf of bread; now it's flat. Pretty snazzy.

Also, I think it might be time to live up to your name and have that nice cup of tea to cure the sleepycranky. ^^

Put quite simply, no. Unless a man is WELL into his senior years, already has that trouble, and is very unlikely to lie with a woman again, the chance of post surgical ED would be a concern for any man on the planet considering this op. I think you really do need to be a male to understand that.

I think you guys are nuts. Take the antibiotics if you have a potential infection. A little course of Flagyl isn't going to change the course of life on earth :) Flagyl isn't broad-range.

If you really think it's Crohn's, then it's crazy to take antibiotics for that.

I was prescribed metronidazole (Flagyl) in hospital just the once. It was the first time I'd been admitted and the hospital thought I had a bacterial infection: they even put me in a sideroom. I hated the metronizadole. It turned my stools green and made them smell like death, and it made me feel sick. Had to force myself to finish the course, then I was back in hospital a few days later anyway.

It wasn't a bacterial infection, it was simply a Crohn's flare. You'd think the hospital might have considered that, given my symptoms and the fact that I already had Crohn's disease.

Those biopsy results have to be ready soon. I am dying to know what you're dealing with. I hope it's something easily treatable!

Flagyl makes me feel like crap too but if you have a bacteria, it can help quickly.

So you haven't considered the possibility that not everyone thinks like you do? Have you heard of asexuality? Or even just that some men would have higher priorities? You might try being more open minded. Which is really what much of this thread has been about. People do not all feel the same way or want the same things that you do.

I'm not sure why different arguments have spawned on this thread. Please be civil, I disagree with what others say but certainly respect their opinions (which is why I invite them to discuss with me). I especially appreciate those who come to the boards to discuss their successful jpouches (esp. ByeByeUC, Pluot, Suebear). I don't agree with many things they say but it is great that they give people on these boards hope.

My OP was meant to discuss the underlying medical idea of a jpouch (specifically, after colectomy, creating an ileal reservoir using ~40cm of terminal ileum plus ~10cm for loop ileo then anastamosing to anal canal) and what the implications are of doing that. I obviously think that there are physiological issues with performing this.

I guess not everybody is considering things on that level.

I do want to say though, I was mentioning the ED thing in relation to pouch REMOVAL. Not Jpouch itself. Any operation in the rectal area runs a (generally small) risk of permanent, complete ED, which I think the vast majority of men at a sexually capable age would consider devastating for more than reproductive reasons. Pouch removal occurs after previous surgery, so a surgeon has to be even more careful when working in the rectal area while there are adhesions. (Original) Jpouch vs. (original) permanent ileo run an equal risk of impotence.

I think that my critiques were fairly objective, as I am concerned with the metabolic and inflammatory effects of creating and having a jpouch. Provided it works "as advertised" I would love my jpouch. If I had a personal issue with it I would not have asked for it.

Most long threads change tack at some point, exactly like how conversations work in real life. Otherwise nobody would ever change the subject and it would get extremely boring after a while.

Also, what more is there to discuss about your OP? I agreed with you about the possible concerns, but said I wasn't qualified to answer them: 4 pages on, that hasn't changed. I actually would be interested to hear an expert address your concerns, but none of us are experts on here, m8: we're just the same old 5 people, banging on about stuff. Nobody here can tell you what you want to know.

She's cranky... but she's right. smilewinkgrin

Totally OT: about these asexuals, where are they? I have never really encountered any, actually i've encountered 2 in my life. I believe they are more prevalent but it's so taboo, way more than being gay - which is actually mainstream now, I think there needs to be an asexual coming out. That would be fantastic.

AnnaIvy: Yes, I have heard of asexuals and had meant to mention them in my post. As for people having different priorities, I defy you to find a man that is sexually healthy with a j-pouch, that would trade pouch deconstruction over being left "in situ", for impotence. And I'm really not sure what I'm not being open minded about, I fully understand not everyone thinks like me, but any sexually healthy male has similar desires (again, I understand, not the SAME desires, not all men are heterosexual) and places high value on functioning equipment. To suggest a man would trade that to get a pouch removed just isn't speaking rationally.

SolomanSeal; I'm sorry you feel things have gone off on a tangent, but NCOT is correct, it's a discussion board so it flows like real conversation. If the pouch really is physiologically bad, then we have a whole heap of neglectful surgeons world wide. It's a shame we don't have a colorectal surgeon on this board.
