I have had an ileostomy for about
2.5 months, all's well but I seem to be losing ground with keeping my skin healthy. I have been discharged from home health care for a few weeks and truly don't think I'm doing anything (wrong) differently than instructed. Even then, it seamed we would get the skin looking really good, then all of a sudden - red and irritated again... the proverbial 1 step forward, 2 steps back.
I've tried a number of brands and products. Started out thinking I wanted a one-piece appliance because of the flexibility. The loop left the top part of the stoma inactive the lower/active part needing a deep convex. I've settled on the Coloplast 2-piece and really like it. Trying to make the flange wear time a little longer because of the insurance allowance so can change the pouch more often. I realize I probably don't have to, it simply makes me feel cleaner. Only problem is now that I'm trying to leave the flange longer, the skin is getting even more irritated. I have tried a combination of skin barriers and whatnot... powder, sprays, barrier strips etc..
I think at least a part of the problem is because the top/inactive part of the stoma pushes over the bottom (it sinks in especially when I bend) and causes stool to go through the barrier ring. Does that make sense? I use Byram Healthcare (AWESOME SERVICE) and they're getting authorization to have more flanges approved, at least for a while. I would like to be able to change it all out every couple days. I've been told that changing to early or often can also cause problems.
Do you have any suggestions or ideas? Your input would be appreciated. I spoke to the surgeon, she said we can look at revising the stoma to an "end" but certainly wouldn't entertain it until the skin is clear and healthy for six months.
