Ileostomates who do not use Eakin Seals?

All Ileo's,

Does everyone on here with an ileo use an Eakin Seal or Eakin Slim Seal?

I lost the 10 lbs that I had overgained and am back in the 120's my goal. Though my stoma was made when I was down to 99 it now sticks out more than when I was close to 140.

BUT I am still getting the burn under the stoma. And prior to my weight gain I did not.

When I first got my ileo I did not use an Eakin as my original stoma nurse did not suggest it. I just cut the convatec Face plate so the stoma had 1/8 or 1/16 of an inch around it so the stoma had wiggle room. I don't remember there being any burns around the stoma as I changed the appliance every 4 days.

BUT I cannot for the life of me remember what we did put in between that tiny gap between the stoma and the cut edge of the face plate (wafer opening). I may have just blown some stomahesive powder in around the stoma.

I may try the appliance without the eakin and see what happens. Seems like the Eakin is not sticking as closely to my stoma as it did when I first started using them and some effluent may be getting under it just a bit.

Let me know if there is anyone on here that does not use an Eakin and how is your skin around the stoma when you do a change. Thanks, Rosemary

Initially hubby used the seals along with the convex wafers as he had lost so much weight and had a noticeable crease that also needed some paste to fill in the ridge. He has since gained weight...some of it was needed and some is not needed...lol. The crease filled out and his skin is now all even so he just uses the light convexity type coloplast wafer now with no seals or paste with excellent results.

With my loop ileo I was using something called Hollihesive from Hollister. It is a square sheet of material that is similar to what the wafer is made out of. I would cut a "washer" out of it which I placed around the base of the stoma before putting the pouch on. I started using this because it doesn't swell or turtleneck at all and still protects the skin around the stoma.

With my loop-end I was able to use a convex wafer and paste only with good results.

RUN4 Aha, That is what I think I need, the Hollihesive. The eakin seems to turtle neck and make it hard for the stoma to push the poop out. Even relatively thin poop is having a hard time sort of jumping over that turtlenecking eakin. I even use the eakin thin and I then squish it even flatter with my hands before I put it around the stoma.

Is the Hollihesive soft and pliable and can actually touch the stoma without hurting it?

Yes, it is ok to touch the stoma. It is hard to describe the material. I would ask for a sample. I took the sheet, cut into quarters, and then cut a hole in the center of the smaller square to fit my stoma. I would put stoma powder around where I had redness, then plop that on top and press it down a little to make sure it was flat, then put on the pouch (I had a one piece at that time). One stoma nurse I had would trim the square edges around the outside to round it out a bit, but another said it didn't matter. I never noticed a difference either way so saved myself a step and just left the outer border square.

Hi Run4,

Good idea to ask for a sample. I never think of that. Seems if I use the powder on the little burn under the stoma then the eakin will not stick to it. Maybe I am supposed to do the powder then some protective wipe ontop of the powder then the eakin then the flange? I could try this with the Hollihesive. Or it sounds like you just did some powder then the hollihesive and the hollihesive stuck right on top of the powder.

Did it come off OK when you went to do a change? Rosemary

Rosemary, not sure what other companies do, but the guy I talk to at edgepark will send samples of a variety of things if I have a particular type of thing I want to try.... you might ask for samples of similar products too (if there are any).

Yes, it typically came off all as one piece with the wafer of my one piece. I guess they bonded together once I put it on? With the stoma powder, I just put it on, dusted off the excess, and then put the Hollihesive on over top. The only time I ever used any kind of wipe was a brief period where I was using the Cavilon wipes to try to heal some bad skin I had.

I tried the Eakin seal with a flat, flexible wafer. It worked for me but left such a mess I stopped using it. I now wear a convatec convex two piece. I put a layer of Friars on the whole wafer area and put the wafer on. Then I put some Friars on the small area between the wafer and stoma. Never had a leak. No irritation. Changed yesterday after eight days. Could have gone longer. I usually go 6 days, but travelling made a change difficult.

You can also call convatec and hollister direct for samples.

I used to suffer leaks really badly to begin with, like 2 x per day using a flat one piece. Tried an eakin seal and was worse the whole thing just came off in a big lumpy mess. Finally switched to a convex wafer and (touch wood) not had a leak since. I simply use a SkinSafe wipe (Cavillon is amazing but I was allergic to it), tiny bit of orahesive powder then the convatec one piece pouch. Lasts 2 days and not had a leak in 5 months. My weight is pretty much the same as it was when I had surgery, although my tummy is much squishier than it ever used to be :-(

Sounds like a good idea to get some samples and tried things out.

Dan is Friers a powder?

Sarah, yeah my weight is down to better, but my stomach is squishier too. I am mid 50's now, though. SO.....

Friars Balsam Compound Benzoin Tincture is the full name. It is a brown liquid much like iodine of the past. It is a first aid product. If you have any open skin it will sting when you apply. It helps healing. It dries sticky and protects the skin. the wafer will stick to it and come off clean when you change. i have been using it for quite some time. It is about $35 for a 100 ml bottle and lasts a long time.

I use the Convatec 2 piece system and use stomahesive paste around the stoma hole on the flange. I stick it on, snap on the pouch and I'm done. This is what they showed me when I was in the hospital when I got my stoma over 9 years ago. I haven't changed the system. It works well for me. I get 5 days before changing, sometimes more... sometimes less depending on my activity. If I sit a lot just after I change the appliance, it tends to slip and break loose sooner and I'll have to tape it. When I change I'll try to either do it at night so that it can set while I'm sleeping or in the morning if I know I'll be on my feet all day.

Brannah, Does the paste go around the hole in the wafer then attach to your body. Or do you put the wafer on your body then squeeze the paste into the area around the stoma between the stoma and the edge of the cut wafer?

The paste goes around the hole on the inside of the wafer then attach to your body. The paste will turtle neck around the hole once its on to create a seal. Don't worry it won't plug off the stoma. Use a nice bead of paste around the hole about the size of toothpaste on a toothbrush.

     Hi Rosemary!  I don't use any seal at all.  I just place the Coloplast one piece right on top of the stoma.  I never have any issues whatsoever with any irritation around the stoma.  When removing the bag (I do this every three to four days), I first wipe with an adhesive remover, then wash the area with a towelette I purchase from a medical supply store (they come in a large stack..don't recall the name), wipe dry and place the Coloplast bag right over the stoma.  Sometimes I have a minor irritation near the outside ring.  I will then put some stoma powder on that, but it is usually clear.  Just make sure the ring fits the stoma closely.

Rosemary, for some reason my skin does not like the eakin seals. Everytime I change with an eakin, no matter how long it has been, my skin will have areas of slight bleeding. It is so weird.

I was using paste around the hole in the wafer, but the last 2 changes, I only used the barrier wipe and that was it. I am using the Convatech moldable, convex wafer and it has been working. No leaks and my skin looks great.

Somebody posted that less is better and I am beginning to believe this is true.

Today has been 7 days for me with no change and when I change, I will let you know how my skin looks. It is slightly itchy and I hope my stoma settles down so I can change it sometime soon.

I did change my bag 3 times this week though so I could have a working filter while I work.

I might go back to trying the paste. For some reason, poop is getting just a tiny bit under the eakin. They do not seem to stick as well as they used to. I did switch from All care barrier wipes to safe and simple (these don't have alcohol). No reason, just did not want to get sensitive to the alcohol in the All Care. As I remember it, I put this barrier right up almost to the stoma, then let it dry and put the eakin over that.

Tracy, those moldables sound great and convex may still be what I need, though I have dropped 10 lbs. The hollister convex did not clear the little burn under the stoma. Does the moldable thing squeeze the stoma? My stoma nurse said those moldables try to reassume their original shape and come right into those stoma's, but she does not have one, so......Let me know.
