I have a problem that is different than most of what I can find on the forum or by a google search. I am a 22 year old male, and I got a colostomy due to perianal complications and pain associated with crohns disease about
3 months ago. Since then I have tried a few different hollister pouches, all one piece, nothing convex, and I cant consistently get more than a few hours of wear time out of them. Every other post I have read complains about
leakage, etc. I don't really notice it leaking, exactly, and it definitely isnt my ouput that is "breaking" the seal. If I put the bag on (standing or sitting, even laying, tried them all) I could wear it indefinitely if I just stayed standing or laying all day. But when I sit down, the sides of the seal "roll" up from skin, making a little n shaped sort of hole through the seal on the left and right sides of my stoma. if I put more pressure in those areas when I put it on, I get the same thing, but closer to the bottom, which has actually leaked before. But most of the time, it is on the side, and it doesn't have output coming out of it, its just that I (and anybody nearby) can smell it, and it is making me miserable.
I have tried adapt rings, but they just transfer the problem to a different area. I have also tried coloplast's elastic strips that are supposedly for this exact issue without avail. Is it the type of pouch I'm using? Or is it an issue with my skin in the area? The base of my stoma is a little raised up, which doesn't seem to help, but my stoma itself is about 1 and 3/4 inch in diameter, and sticks out from my skin almost as much (which seems huge to me, is that normal?). I am also having leaking issues due to a wound near my stoma. The ostomy nurse said something about a mucosa fistula I think, but whatever it is, it is about the size of a pinky finger fingerprint, and it is covered in tissue similar to my stoma, as opposed to skin. Do I need to see a surgeon to have that fixed? It puts off mucous and makes the area (near the top) leak sometimes (as opposed to my other, "rolling up" issue at the sides that isn't "leak"ing.
Sorry to throw so much into a post, but I really should have looked for help a month ago, before I was back at work, but I just kept hoping it would get better in a couple weeks, and here I am still having the issues. I have done most of the obvious. No lotions or soaps except ivory in the area, i scrub it with a warm wash cloth to clean it, then dry it totally before i apply the bag. I have cut to size pouches that I cut to 1 3/4 to 2 inches, I can fit it when I cut it to 1 1/2, but it is pretty tight then, and the stoma nurse said that the skin barrier was cutting my stoma, which created\exacerbated the mucosa fistula issue.
