Ok Guys.....  Just got back from the URO... got the cath out (Thankyou GOD!).... couple of drips for now... nothing major. I'm sure I'll be able to assess this better in a couple of days. Other than feeling a little soar after the cath removal.

I'm actually doing very well.  Soreness around the stomach is 85% gone.

But I do need your opinion on the path report guys... please tell me what you think:

Total weight 50 grams 

Gleason downgraded      3+3=6 

Tumor Involvment            15%

Subcapsular Perineural invasion  -  Present rolleyes

Lymphnode - Negative 

Distal Urethral Margin - Negative

Radial Margins - Negative

Seminal Vesicles - Negative

Bladder neck margin is Postitive with a 1mm focus (unifocal) smhair

Final grade  pT3a N0 MX

I'm no pathologist.... but everything looks good except for the 1mm protrusion into the bladder neck.  I spoke with the pathologist and he did mention that that area may very well yield nothing in the future do the the sterilization that occurs in the nearby tissue dur to cartorization during surgery.  I guess only my PSA's will tell which direction I'm headed.

darn good thing I didn't wait based on my 2.75 PSA huh? Your thoughts would be greatly appreciated

hey vito, the gleason downgrade is always a nice thing. like yourself i had a very small pos. margin in one spot and like you was told it will likely not have a adverse effect on my recovery. good luck to you, tim

Looks good - glad that is behind you -  your doctor gets the pathology report done quickly - i had to wait several weeks for mine

I believe you are correct, your upcoming PSA results will indicate if additional treatment is required.

Amazes me to see people like yourself with biopsy results of one positive out of 12. Thanks God that one needle hit the tumor if not would not have known.... And with just one postive still Stage T3!!

I had the same downgrade of Gleason from 7 to 6 which I here from other posters could be helpful long term.... Take it easy!!!!

The Gleason downgrade was great. The bladder neck - not so great, but I'd still trade Gleasons with you.

You'll know about the leaking tonight - if you don't have to get up during the night and aren't swimming when you do wake up, that's a really good start.

The down grade from Gleason 7 to Gleason 6 is the best news. So is the smaller tumor %, meant it was smaller than they thought. The Stage 3 part is not good on the surface, but due to the small of the cancer and least agressiveness with the Gleason 6, you should be in good shape. I am sure you were glad to be rid of that catheter. Good luck in the next phase of your recovery.

Yeah the Gleason score is a good thing.... the T3a is a scary one... but when I put it in perspective, the staging has to be that way even if there was a micron of protrusion.  Doc said it was 1mm, but he's not concerned and that I shouldn't be if he's not. 
Still scares me to think and this "thing" was able to chew through given the low % that was involved. Given the fact that everything else is clean... should I be really concerned???  I'm not going to dwell on this, or at least try not to until my first PSA comes back.

Cath felt great to have out... although I'm getting a tremendous sensation of urgency once it empties. That may be due to the cath just coming out. I'll know more after a couple of days when everything settles in.

While there is good news on the Gleason downgrade, you are not the first person I have seen here that had numbers low enough to feel good about active surveillance yet come out of surgery with something totally different. But I am with your doctor, it's time to watch and wait with hope of stringing undetectable PSA's. Good luck in the coming weeks. My motto is stay positive and I know it has done well by me. So do so and get on with your life. Keep us posted on your progress...

Thanks TC...one thing I have been noticing also... is that people with positive margins, still pulling a T2 path result.

I was under the impression that only T3 meant that there was protrusion

Congrats!.........yeah, the path results are mixed, but certainly more positives to hang your hat on. Like you said, the subsequent PSA checks will tell the story. Concentrate on the recovery now that the snake has left town.

Vito: I was one of the lucky ones like you downgrade from 3+4 to 3+3...That does not happen too often. Sounds like you are on your way.

Lake all of us even with good reports PSA watching, PSA watching.

Yes a path report like yours is a bit of a mixture.
I think the best thing will be to ignore it and wait patiently for the PSA test. Concentrate instead on continence and recovery etc.

Ok folks..... day 2 after cath removal... (don't worry, I won't be giving day by day blows)

Pretty dry over night except for a couple of drips. Woke up twice with bladder pressure.  Funny enough... stream seems to be the same as pre-op.  Only problem seems to be the pressure I'm getting toward the end of the stream when urinating. It actually stops by itself, the pressure comes then I actually have to push a little to get the rest out.  I'm assuming thats from having the cath in for a week.... I'll give it the weekend then I'll give my uro a call if it pursists.  Haven't allowed Johhny to go out and play yet.

I was a 6-6 Gleason with a 1mm focus on the margin. My doc wasn't worried about it so I decided not to worry either.

Keep walking and kegeling, and why not ask for an ultrasensitive PSA test at 6 weeks to better chart your "undetectable" !?!?

Surely there are many of us that would like to trade spots with yours or have your stats, the theory of relativity-even if not related to you- (LOL). It does look pretty encouraging for you overall, look at R&R activities.
