During the few weeks since my diagnosis (12/21/12), I have read Patrick Walsh's "Guide to surviving prostate cancer (2nd edition, 2007), Peter Scardino's "Prostate Book" (2005), Eric Klein's "Prostate Cancer" (2008) and Arnold Melman's "After Prostate Cancer" (2011). All of them are written by surgeons who consider the surgery the gold standard. I could not find any book written by a radiologist.
So far, I have met a surgeon at the University at Michigan (who recommended surgery) last week and I will see a radiation oncologist at U of M and a surgeon at
Henry Ford this week, followed by a CyberKnife radiologist appointment at St. Joseph the following week.
My goals: 1.CURE CANCER 2. Maintain continence; 3 Minimize other side effects.
Based on my understanding of my cancer and using different source, I compiled questions (pertinent to my case) to ask doctors; they may help others to assess their case.
DIAGNOSTIC
55 years old, 5' 11', 170#, no symptoms before PSA testing in October 2011. Physically active, no other disease and no additional medication other than lorezapam (0.5 mg) started after diagnosis (only as needed to contain my cancer-related anxiety).
"Biopsy results (biopsy 12/15/11, results, 12/20/11):
T1c, low grade, Gleason score 6 (3+3,50%/50%), 2 of 12 needles positive:
Needle 7 Left lat apex, pattern 3+3, < 5%
Needle 8 Left med apex, pattern 3+3, 5%"
Where is the cancer located (peripheral, central, anterior, transitional)? How confident are we on the Gleason score reading (3+3)? For the confidence in the risk of the cancer, should I get another biopsy examination?
How close to the margin (peripheral border of the prostate ) were the positive biopsies ?
What are my PSA readings telling about the type of cancer? (2.8/09, 3.4/10, 4.2 & 3.7/11)
Do you think there is any Seminal Vesicle involvement or lymph nodes?
What do you think about the size of the prostate, 44.6 cc? How can I get the ultrasound recording?
What is the likelihood to be a multifocal cancer?
Are there additional tests that we can do to gain the most complete understanding of the stage, spread and aggressiveness of my cancer MRI, CAT or bone scan)?
What molecular biological marker should be examined to better predict the aggressiveness of the cancer? TMPRSS2:ERG and PCA3 DNA (prostate cancer antigen) are expressed at high levels in 95 percent of prostate cancers.
What would you estimate are the chances that the cancer is contained within the prostate Partin tables)?
Could my GI symptoms be related to my diagnosis? Or the slight discomfort in the right side?
TREATMENTS
Dr. (surgeon, radiation, med oncologist), what are the treatment options and what would you recommend?
Based on your experience, why is this the right approach for me?
What is the probability of my cancer recurring with the treatment you propose (%)?
What are the benefits of the type of therapy you are recommending?
How many procedures of the treatment you propose have you performed (total and yearly)?
Would you recommend active surveillance?
Are there dietary or lifestyle changes that I can make that can slow prostate cancer growth or that may allow my body to be more healthy to fight this prostate cancer?
ROBOTIC SURGERY
If I select surgery, would you recommend nerve sparing for me (unilat/bilat)?
How many procedures have you performed?
What are the positive margins for your patients?
What are the cances that I will be cured (cancer free, >10 yrs)
What will we do to monitor my prostate cancer following the surgery?
What can I expect following the surgery in terms of recovery time? How long will it be before I can return to my normal activities?
What level of success have you had in preserving urinary continence and nerve sparing/ED (%) at 6 month and 1 year?
RADIATION
Are there specific radiation therapy approaches that we should discuss or consider, such as 3D IGRT, brachytherapy, proton? What about fiducial markers to help guide the radiation dose in real time to possibly prevent toxicity (Calypso, CyberKnife)?
Do you have both External Beam and Seed Implant Radiation ?
How long has he/she been doing Seed Implant Radiation ? How long External Beam Radiation?
How will this procedure precisely target the cancer tissue but leave the normal tissue unharmed?
What dose of radiation will you be using and how/why did you select that dose of radiation?
What about preserving urinary and bowel continence (%)? Are there other urinary or bowel side effects that I should be concerned about at 6 moths and 1 year?
What is level of success in preserving potency in your patients following this type of radiation therapy?
Do you recommend that we initiate androgen deprivation therapy ("hormone therapy") before the radiation treatments? Why or why not?
How often will I need to come into the clinic for treatments?
What can I expect following the treatments in terms of recovery time? How long will it be before I can return to my normal activities?
Are there delayed side effects that might appear over time (after 3 years, including cancer in another organ?
What will we do to monitor my prostate cancer following the radiation?
Are there clinical trials involving radiation therapy that are reasonable for me to consider?
