Trimix first, what a mess!

Michael went to his URO for his first Trimix instructional appt. He took the trimix and a syringe. The Dr used .2 (and that is correct, not .02) . Within 5 minutes he was rock hard. At 3 hours, he contacted his dr again still raging hard, he then called in terbutaline sulfate 5 mg. Take 1 every hour for 3 hours and if it hasn't gone down then he needs to head to the ER. At 6 hours, he decided he needed to head to the ER, after taking his 3rd pill, he got in the car to go. On his way he felt a bit of relief, so he decided to wait a bit before heading in. After about 20 minutes, he was finally soft again. He didn't end up going into the ER, and had a rock hard erection for 7 hours and 20 minutes. He did ice, and he showered, and he laid down....I told him to go get Sudafed, but the DR wanted him to take this other medication instead. At about 4 hours he was saying "I wish this would get soft" and when it was finally gone he said "Thank God for a soft c!!K!" What a mess. I was under the assumption that the Drs started low and then slowly added more till desired effect. I guess he was having a lapse in judgment to start him at .2

At this point, he is scared to try again! I can understand, and assume we should wait to try again. The DR stated he wanted him to try .1 next time we do try. Which he stated could be used every day!

Scary Trimix experience!!!

My doc started me at .1ml and it lasted about 20-30 minutes. I wasn't hard enough for intercourse.  Three days later I went to .13, and it was better but not great.  three day later went to .16, and now we're getting somewhere.  A few days later went to .18ml and that's where I'm staying...... comes up in 10 minutes and lasts 45 minutes to an hour rock hard. Goes completely down in an hour or so.

This stuff is powerful, and I'm so glad my doc didn't start me off at too high a dose or I may have never been interested again. 

My doc told me to plan my activities during his office hours until I get the dose right.  Instead of going to the ER he told me to call his office if my erection lasts three hours then start heading his way, and at the four hour mark he'd give me an injection to reverse it. Too bad your doc didn't offer you the same deal. 

Don't give up.  It's a great thing when you get your dose right. Good luck, hj

I started at 0.1 mL per dose about 9 months after surgery. The prescription was pretty standard for Trimix.

I had a trip to the ER. Sudafed, ice and a caring ER doc got things settled down without having a procedure to withdraw the erection before long.

My last dose of Trimix was at less than 0.025 mL. That is very low. Even at those low doses I had a couple close calls in terms of 3 1/2 hours plus. I stopped Trimix and now Levitra at low doses (5 mG) is 100%.

My Uro ED doc said I was lucky and would probably respond to low dose orals...which I do...and often don't need. Maybe Trimix got his gear working again. It happened for me. Is your husband seeing a URO ED doc that specializes in ED after RPP or other treatment?

When my doctor prescribed Trimix he started me on 0.1 first and told me that if that didn't work to increase it to .15 the following day.

wow susan, its a miracle that a "woody" that lasted that long didn't cause some real damage, can't even imagine that. glad it had a happy ending. good luck on his future attempts.

I had inconsistent results and worked up to .35 ml . I dad several 4 hour erections and one 6 hour that Im convinced did some permanent damage. i.e. shortening, curving and girth loss in the distal 3rd of penis that developed several weeks after the 6 hour ordeal. My dose now is .05 to .07ml but the deformity remains. Be cautious and don't let it go on that long. An implant is likely the only thing to resolve whats been done to my unit now.

Susan & Michael – what an awful experience!!!   I cannot believe the Dr. injected Michael for the first time with 0.20!!!   And then sent him home before it went down???   And when you called at 3 hours, he called in meds, but said not to go to the ER until 6 hours???   I hope I’m not breaking any forum rules … but (IMHO) that’s insane!!!     I’m seeing a highly respected ED specialist.   His training consisted of two appointments on back to back days.   Day one, my first injection, he started me at 0.05ml, administered by his NP in his office; I got to about 40%.   Day two, I gave myself the shot, under the supervision of his NP to make sure I had the technique down; this was at 0.10 based on the results of day 1.   Day 2 got me to about 80%.   On neither day was I allowed to leave until I was well below 60%.   After the second appointment, I was sent home with instructions to try 0.07 – they  believe that there is a certain anxiety level for your first injections, especially in a doctor’s office in the presence of the NP, basically a stranger.   So they figure you will get a better response at home.   I have since settled on 0.20 as the dose that’s right for me – yes the same dose that Michael was started on.   However getting there was a very deliberate, intentionally slow process with strict monitoring by my doctor.   It took a month.   I went from 0.07ml, to 0.08, then 0.10, 0.11, a “big” step to 0.15, 0.18, and finally 0.20ml.   I had to check in by phone, with my results to his NP along the way.   I only increased the dose under his instructions.   And each dose was tried at least twice , just to be sure of the results, before going higher the next time.   I am still under strict instructions not to increase the dose with out talking to the NP or the doctor first.   All this being said, everything is relative to the strength of the Trimix being used; so you have to compare the mix.   My Rx is Papaverine 30mg - Phentolamine 1mg - PGE1 10mcg/ml.   Regarding waiting 6 hours to go to the ER: my doc says a 4 hour erection above penetration hardness is considered to be a urologic emergency.   Penetration hardness is considered to be an erection of 60%.   So anything above 60% is cause for concern.   This situation deprives the penis of oxygen and that can cause a degradation of erectile tissue.   For an erection lasting 2 hours, you should take 4 Sudafed 30 mg tablets (regular Sudafed not the extended release) to knock it down, if it’s still up at 3 hours you should be in contact with your doctor, and if you’re still 60%+ at 4 hours you should be in the ER.   I certainly appreciate Michael’s reluctance to try again.   I’ve thankfully never experienced a trip to the ER and my maximum duration is 1 hour 5 minutes, so I don’t know how long he should wait to inject after his experience, but normally you need to wait 24 hours between each injection, and you can only inject a maximum of 3 times a week.   When he gets the courage to try again, IMHO he should try 0.05 or even less.   Sorry I got so long winded … and I hope I didn’t get too preachy; I’m just really alarmed at your scary experience, and I’m trying to help.   I hope you get this worked out so that you can both enjoy the “recreational” and rehabilitative benefits of Trimix.   All the Best for success in all aspects of recovery.

After reading many injection postings on this forum and also on FrankTalk it seems like the uros are a bit out-of-touch when it comes to initial injection drug dosages. I think it would be a valuable learning tool for the docs to check out the real world experiences many of us have endured via these postings. When I first attempted to use Trimix I was told to use .2ml (20 units). On my own I decided to reduce that amount to .15 ml (15 units) and gradually increase if needed. That first iinjection lasted a little over 4 hours and subsided only after getting dressed and preparing to get into the car for the ride to the ER. I had already tried walking a mile on the treadmill, Sudafed, ice bag and cold water shower aimed at the affected region. I think the fear of the impending ER trip released enough adrenalin to finally bring it down. Right now I'm using .08 ml (8 units) of Bimix. Trimix worked well but wasn't acceptable due to the alprostadil (prostaglandin) pain.

Sorry about the bad first experience with Trimix. Trimix dosages seem to be different for everyone of us and really  there isn't any standard dose. I now use a double strength version at .35 ML which took a while to find that gives me the perfect 1 1/2 hour erection. I started out last year with the standard strength Trimix at .3ML and got up to .75ML before my URO switched me to the double strength that costs less per injection. I hope you have a better second round.

After it is over for a few days, some of the humorous aspects of having an erection like that, and going to the ER makes it not so bad.

Jay Leno quipped that before he went to the doctor, he was going to call all his friends to come over and see that sucker.

Hopefully the next time a much lower dose will work out to your mutual satisfaction.

Thanks for the input. Michael was able to achieve orgasm with semi-erect penis. So we know he still works. Thank God!!! He is willing to try .1 tonight. Will let you know!

Yes, I am thrilled he hasn't given up. Am excited to feel the difference!!! Ooops, guess I shouldn't have said that out loud. 7 hours and 20 minutes of erection on .2, so half that......approximately 3 hour erection....darn....I can only dream

Susan, drive your motorhome and park in the ER parking lot.  Draw the curtains tight, and if everything goes well, you can drive home later.  If not, hubby can walk to the ER....... then you can uh...drive home later... 

Don't laugh, I was thinking about it while trying to get my dose right..... but sweety was afraid someone would see the curtains drawn and hear the RV shocks bottoming out.  shocked

I think David's "happy ending" comment was more that thankfully nothing fell off. I had not heard that term in a sexual context until the other day on a "CSI" type show talking about an overly ambitious masseur looking for extra cash. Granted I spent most of the last 25 years outside the US, and make a point to avoid certain illnesses and jail, so perhaps am completely oblivious to a lot of the hip(?) terminology.

I started at 0.1, and the uro did it. I had to wait in the office until it started to subside. I went up by .01 (.10, .11, .12, .13) until the .13 had me packing for the ER with a vial that was 4 months old )officially expired). For now, the dose is .11, and I'm "sticking" with it.

Ok, not so good time by ourselves! He shot .1 and we waited 15 minutes and nothing happened. He felt odd about doing it so wouldn't let me
Participate but when I asked if he pushed te needle all the way down he said no. So I think he just didn't push it in far enough. He said there was a slight burn when the liquid was going in and i think he was just plain old scared about trying again, will try again in a couple of days. He needs to push the needle down all the way, right? We are hoping the little needle prick and the slight burn will end up being worth it at some point!!!

Yes he should push the needle in so the syringe body is touching. In fact with 5/16" needles I often find I need to push enough to dent the body into the skin. How can you tell if needle is in far enough? It will be easy to depress the plunger if the needle is all the way in the CorpusC. If it not in there -- maybe just pushing against the membrane of the CC the it will be hard to depress the plunger on the syringe (in that case you can try continuing to jab the needle in some more to get into the CC). Although the doc-approved method is to JAB the needle in with one swift movement (and it certainly works that way fine) I prefer to slowly do little jabs until all the way in -- cause I am a masochist!. But you really get the feel for when you are in the CC that way -- plus the plunger thing.

Susan, just a suggestion, I understand the drug store sells autoinjectors.

According to what I've read, you load the syringe put it in the injector body, position it and push the button. Instant injection. No thought process or coordination involved. That was going to be my next step if I found I couldn't do it myself.

Also, maybe some practice on something would help. When my daughter in law was going through nurse training she used a syringe filled with water and practiced on an orange. In your hubby's case, maybe a kielbasa?

I touch the skin take a deep breath and briskly stick the needle all the way in and slowly push in the plunger. No pain but I do feel the juice going in. The worse thing about the whole ordeal is the thought of it. good luck, hj

I bought an auto injector from Amazon online as I thought that would be the only way I could ever inject myself. I used it one time as I found it to be not a problem self injecting.

I bought an auto injector when we got the RX for the Trimix, a few weeks before the instructional appt. I had forgotten about it. I will go try to find where I put it! Maybe doing it that way will help get over the "I am putting a needle in my ....." I bought it off Amazon. Will let you know.....not sure he is going to try again today, and then he works 12 hour days for the next 3 days, so will be a few till he tries again. In the meantime, let me know if you have any other tips!
