Berdo:
I had somewhat similar stats (G 4+3) and chose surgery (by a world-class surgeon). I don't regret my choice one bit. Here is why I chose surgery. But keep in mind I was 63 at the time and in excellent health otherwise.
Let me point out that the outcomes I wanted in order of importance were:
1) Cure the Cancer (way out in front -- unfortunately I we failed in that)
2) No incontinence (succeeded totally)
3) No ED (a distant third -- I'd say somewhat succeeded)
The reasons I chose surgery:
1) SE are immediate but will get better. I did not want to have to deal with bowel or other problems a few years down the road and those problems might not get better.
2) If surgery fails, I do have SRT to fall back on (unfortunately, surgery failed and so did SRT)
3) With surgery, I get it out and they can do a thorough pathology so I have a very good idea what I am dealing with.
4) Surgery will still debulk the tumor. In my case, my PSA went from 4.19 to 0.01. Unfortunately, it did start climbing soon thereafter, but I think starting at 0.01 did buy me some time.
Anyway, that's just me.
In terms of the surgeon, I researched it a lot and narrowed it down to two. One was robotics, the other did either. While I preferred robotic surgery, I was totally going to choose based on who was the best surgery, as best I could determine. Incidentally, I did meet extensively with a radiation guy too. In fact, it was the same day, at the Univ. of Michigan Comprehensive Cancer Center. That day I met with a surgeon, then a radiology guy. I was then supposed to meet with a prostate cancer oncologist (Dr. Hussein, who I eventually did see a year later) but cancelled that as all my questions at the time were answered. The next day we met with the other excellent surgeon, Dr. Menon. He did my surgery. No regrets. I had ZERO complications and recovered well.
Unfortunately, in my case the PC may do me in...BUT NOT TODAY!
Keep in mind that I am not recommending any specific modality. I'm just communicating my own reasons.
