Doubling time - PSA

Husband got off of HDT in April?.  His PSA was <0  Then here are the PSA results:

Aug .18; Sept .34; Nov 1.02; Dec 1.96

Doesn't look good .. does it?  Dr. Sartor appointment on Tuesday, but husband is starting to freak out.  Any thoughts of encouragement yall can give me to give to him will be greatly appreciated!  And .. what do you think will be the next step?  He's done HDT and radiation ... thanks

Has he had his testosterone measured too? It looks like he was on hormone therapy for 2 years, so it may just now be recovering to normal levels. While the PSA trend is worrisome, it may not necessarily indicate a problem. It may rise a ways as the testosterone does too.

If he's only had radiation and hormone therapy, it's not surprising his PSA would rise to some level as his testosterone recovers. It should come up, stabilize, even wander up and down a bit.

Post radiation, there are various definitions of recurrence, but a common one is 2 points above the nadir. But, I think that nadir is only valid if you're not on hormone therapy since it will artificially suppress PSA anyway. After you stop hormone therapy is the only time you find out how much good the radiation therapy did.

Others more knowledgeable will be along shortly I'm sure to add or correct, but I'm thinking it may not necessarily be the frightful thing you're perceiving right now. As a fellow G9, I understand to some degree the fearsomeness of those renegade cells, and can see why you're concerned.

Oh, and next step? I'm sure he'll go right back on hormone therapy!

My guess would be back on HT, possibly permanently. why would they do radiation when it was obviously metastatic. the length of said treatment indicate , primary treatment was radiation. De bulking I presume. If the testoerone is going up with psa right behind it, that is not a good thing. My "knowledge" tells me if radiation is used for debulking and not a cure, which with mets, is out, the nadir is not even part of the discussion with a medical team. Please for the threaders sake, correct me if I am wrong.

hopefully more of the G9 crew will be along soon. Sounds like he needs to get back on HT. It's not a one time thing, he may been to be on it or other meds for a long.

I'm part of that G9 crew that Andrew is talking about. I just came of my second triple blockade vacation with each one only lasting 7 months. Also my PSADT is about 2 months. Dr. Lam had me try Metformin and Celebrex to help extend the vacation time but to know avail but it might work for your husband. The good thing is that it seems is that he is not CR so the hormone therapy should keep on working. It seems that his PC like mine is still very dependent on Testosterone.

I'll go ahead and bump this to see if Debby can get a bit more advice. On the caregiver's thread she just posted a new PSA (1/22/15 - 7) and that a recent CAT scan showed mets to the lung.

Since his cancer waited seven months after he discontinued his androgen deprivation therapy it seems likely that he has not become castrate resistant.

Back on ADT-2 until it quits working, then Provenge or Xtandi..

Are you into supplements? Try Pomi-T It's inexpensive and has good reviews...

Sounds like his HT Vacation time might be over with for now, be a good time to hit it again hard, before that PSA number starts to get out of hand

Agree that its time for HT, very likely permanently. But that should get it fact to where you want it . If not many more tx's in pipeline, but they are above my pay grade and do not want a promotion…ever.

Update ... PET scan showed negative for mets in his lungs turn   after all.  PSA has doubled again ... and is now close to 8 ... so he will be going back on HT this coming Tuesday.  That's ok .... what we were expecting was a whole lot worse!
