My husband is 72 and has been healthy.
In the spring he had his first PSA after difficulty urinating and a bumpy prostate on exam.
His PSA in April was 25.61.
He had 12-15 biopsy punches and they were all normal.
His PSA in 7.69 in September after being prescribed finasteride. And up to 9 right before the MRI five weeks ago,
He had the MRI, with gadolinium and an endorectal coil, done and there was a tumor visualized.
From the MRI report: "There is a 1.5 x 0.6 x 1.5 cm T2 hypointense nodule in the
right mid gland and apex. There is bulging of the right prostatic contour with apparent nodular
foci, likely representing extracapsular extension that abuts the neurovascular bundle. There is associated increased perfusion and restricted diffusion. The seminal vesicles are unremarkable.
PROSTATE VOLUME : (6.6 x 3.2 x 4.6 )/2 = 48 cubic cm
BLADDER: Normal.
RECTUM: Unremarkable.
LYMPH NODES: No pelvic lymphadenopathy.
IMPRESSION:
1.5 cm mass in the peripheral zone of the right mid gland and apex
with likely extracapsular extension, which abuts the right
neurovascular bundle."
I missed the last urology appointment because I was recovering from knee surgery. They scheduled a high tech biopsy, I think with MRI guidance. Not sure how that works, but a younger urologist is doing this test.
Husband is a physician and is being treated at a big ivy covered teaching hospital in the North East. We trust the pathologists and physicians.
We've been happy together for thirty nine years. I'm not sleeping at night and worried sick about him. He was OK until he read the MRI report, he's not happy that the presumed cancer seems to have grown out of the capsule. He's still working in a specialty that allows for a lot of sitting around. His first impulse was that he'd have to retire. I support whatever he wants to do, but he's a bit of a workaholic, and is always busy.
So what next, Guys? I realize that Gleason scores and treatment plans are coming. I want that sucker outta there, but I've read that surgery isn't "done" after age 70. And any treatment plan is his choice.
He needs time to recover from either surgery or radiation. I'd also like to know what to expect if we get a bad Gleason as well as growth out of the prostate and possible mets. And what kind of life, or not, are we are facing?
