With the psa going down and a 49 gm prostate and state of the art equipment. they may find some minuscule amounts of PCA, so you could hear the dreaded word CANCER. Don't sweat it. they may find zip, but don't sweat it either way. Love the City, lived there 8 years in the 70's. Ah Yes the City…brings a tear to my eye. Good Luck

I got the results already.
The biopsy itself was a breeze. No pain at all. I did take 15 mg of Valium, so I know that helped.
14 cores taken, with 8 reported.
All results negative. Results below.
Everything looked normal, nothing unusual.

Prostate size 45, prostate density .8
But, since there was no blood flow for the color doppler biopsy to pick up,
isn't this really just a blind biopsy with a 70-80% accuracy?

So I am seeing if I can get a MRI to get more conclusive proof.
I don't like the idea of sitting around and wait to get more psa tests.

So I am also taking some supplements to lower my psa that I found out about through
Life Extension Foundation.
Any suggestions on non drug ways to lower psa or drugs with minimal side effects?

Good news. Your prostate biopsy came back negative. Dr.Shinohara recommends continuing to monitor your psa at every 6 month for now and please contact me us if you have any question

Relevant history according to APeX: The patient is a 61-year-old man
with elevated prostate-specific antigen (3.8 in 12/2014).


Gross Description
The case is received in eight parts, each labeled with the patient's
name and medical record number.

Part A, received in formalin and additionally labeled "right apex,"
consists of two 0.1 cm-thick cores of soft tan-white tissue. The cores
each have a length of 1.3 cm and 1.5 cm. The specimen is entirely
submitted in cassette A1. (lds)

Part B, received in formalin and additionally labeled "right mid,"
consists of two 0.1 cm-thick cores of soft tan-white tissue. The cores
each have a length of 1.7 cm and 1.2 cm. The specimen is entirely
submitted in cassette B1. (lds)

Part C, received in formalin and additionally labeled "right base,"
consists of two 0.1 cm-thick cores of soft tan-white tissue. The cores
each have a length of 2 cm and 1.6 cm. The specimen is entirely

The attending pathologist has reviewed all dictations, including

OPERATION: Transrectal ultrasonography of the prostate and ultrasound guided prostate biopsy.


CLINICAL INDICATIONS: 61 y.o. man who presents with an elevated PSA.


The patient's PSAs are as follows:
12/14: 3.8
10/12-14: 4.6-4.9
3/2008: 3.1
11/2006: 2.1


He today presents for a biopsy.
Digital rectal exam revealed an enlarged prostate, smooth without any palpable lesions.


FINDINGS: Serial transverse and longitudinal scannings of the prostate gland were obtained. The prostate gland was estimated to be 45 cc and transition zone is 19 cc.
No hypoechoic area is noted. Prostatic capsule is intact. Extracapsular extension was not noted.


PSA density of 0.08


PROCEDURE: The patient prepared for the biopsy with pre-procedure antibiotics and an enema. After informed consent was obtained, the patient was taken to the procedure room and placed in the L lateral decubitus position. A digital rectal exam was performed. The ultrasound probe was then inserted into the rectum and serial transverse and longitudinal images were obtained. The prostate volume and the transition zone volume were calculated. The prostate and SVs were inspected for areas of hypoechogenicity.
The probe was removed and beta-dyne was used to prep the rectum. The prostate was then anesthetized with 20 cc of 1% xylocaine. Using ultrasound guidance, 14 biopsies were taken, with 1 biopsy from the anterior, apex, mid, base in the medial and lateral portion of the prostate bilaterally. At the conclusion of the biopsies, the rectum was inspected and appeared free of bleeding. The patient tolerated the procedure well and released from the exam room uneventfully.

IMPRESSION:
Digital rectal exam revealed an enlarged prostate, smooth without any palpable lesions.
The prostate is 45cc, the transition zone is 19cc.
There was not a hypoechoic lesion noted

Ok, am I reading this right that you're 61 w/ a last PSA of 3.8? Isn't that still in upper range normal for PSA? If the prostate is enlarged that could account for the ED issues. I can't tell you what to do because I'm a worrier so I'd want more testing but I'm not seeing a lot of red flags either. I want to say that our first urologist had told hubby that if his PSA was 4.1 and he was in his 60's or 70's he wouldn't be as concerned.(This was the urologist we fired for other reasons so take if from the source it came from). If you're not comfortable waiting I'd pursue further investigation. Yes I rambled and some more of the smart people will be along shortly. ~Miki~

Hi jgcec! sorry you have to be here and glad it looks like you don't have to join our crappy club!

So your PSA has been dropping? From 4.6-4.9 to latest 3.8? Plus your prostate is enlarged, which could account for any rise? And your density is only .08? And your DRE is negative for nodules? Frankly, I'm not even sure why a Bx was done, but I am probably missing something. Was it done based on the 3.8, or the earlier slightly higher numbers? Anyway, it was done, and your Bx is totally negative?

Well, anything is possible, but I know what I would be doing if it was me, and that would probably be nothing other than occasional PSAS. Of course, folks are different. And also depending on the risk and possible SEs of any further testing, like Bx. I'd just be considering myself off of the hook. And like you are doing, I would pay attention to diet and such. Lots of studies out there on stuff that might be helpful, maybe.

Well this should be interesting!

Bx I guess means biopsy was totally negative, 14 cores total.
and was done using a color doppler ultrasound at University of California San Francisco. UCSF is one of the few places that does color doppler ultrasound and Fusion MRI for targeted biopsies. Other people have had the MRI first, so that is probably my next step in a few months.

DRE was done twice by different doctors
and both times there were no abnormalities of any kind.

I had the biopsy because my psa was 4.9 for 2 years,
and just recently went down to 3.8 two months ago.
That 3.8 could have been wrong.

Also I have read of men having PC with much lower psa readings.

My free psa test was 13%, which worried me.

I would like to get my psa down to 1.

I have read about 5 books on PC and they all say how elusive pc can be.
So maybe I have read too much and I am too worried about it.
