Husband's prostate cancer back?

I'm terrified. My husband had prostate cancer 3 years ago and it was PSA >50 and Gleason 7 (4+3). He was treated with implants and radiation and PSA has been and still is extremely low. He went for an MRI due to back/hip pain and a lesion "worrisome for neoplastic process" was on his lower spine. Now we have tests to wait for. Can he have mets with an almost undectectable PSA? Is there any other explanation for a suspicious lesion? If it is CA again what is prognosis? I know he will have to go on hormone therapy again which nearly killed him last time (he stopped sweating, couldn't walk, so fatigued he couldn't do much of anything). Will he have other options?

There are a lot of old injuries and anomolies that can show up on scans.
If it were pca, spot radiation might be a treatment.

I am not a doctor and can only speculate. Do you know what the PSA number is? If it is "almost undetectable," I'd be be surprised to learn he has PC mets. I would expect mets to be preceded by a rising PSA.

There are a number of things that can cause back and hip pain. I know it's not easy, but you'll have to wait for the test results.

If it does turn out to be PC again, there are a number things that can be done. Possibly spot radiation to the met, HT, chemo. Far to early to speculate, but I suspect they could treat it.

Did he have a bone scan before he had the seeds and radiation? With a PSA that high back then, I'm assuming he did. Just wondering if the lesion, whatever it is, was there earlier. Could be anything. Mets are highly unlikely with an almost undetectable PSA if he's not on hormone therapy. I hope the biopsy is negative. Let us know, OK?

Thanks for the responses. No he had scan pre treatment and this lesion did not show up then. Doctor says, "most likely mets." He's going in today for total body bone scan and then MRI of pelvis rather than just his spine. Scary. I guess you are right, I need to wait. He is scared too, just doesn't react like I do . . . panic. Then doctor is off all next week and doesn't seem to think it a big deal to make us wait for results. The thing is my husband doesn't want me to tell anyone until we know for sure. So I have to pretend this isn't happening (except here). Thanks for the support.

Dandy, the waiting is always the worst but as you know not much you can do about it! Just try and hang tin here. It can still be something else. My mets are small lesions but were kicking out some serious PSA and G9 to boot which, produces less PSA normally than say a 7. With his low PSA I wouldn't get into the panic mode yet! We're always here if you need to vent!

If it turns out to be a met, and it's just the one, they might (depending on location) be able to zap it with 3-5 doses of SBRT. You can deal with anything you know about - the problem is when you don't know, and the mind spins in circles trying to deal with projected scenarios. Really it's pointless and exhausting, though hard to stop.
