It's amazing but most of these forms leave very little space to write this in. I could say "see attached sheet." But I also see the advantage of being brief, as one can't cover every conceivable situation.
...which is exactly why you don't want to do that. If you take time to detail out many scenarios, but not the infinite number of scenarios, then you have created reasonable doubt about
those situations you specifically left out. As I said previously, the objective is to provide "general guidance to be applied to the infinite array of possibilities." You should also TALK to your loved ones (specifically, those you have designated as having power if you are incapacitated) about
what you have written and your intent to have a dignified death.
