Ok I have to admit that I am having a rough time I have selected my treatment that starts 7/26/2016> I am doing the : EBRT with IMRT and daily IGRT + long term (2 years) ADT.

I also know that Per MSKCC prostate cancer nomogram, which draws on data from > 10,000 prostate cancer patients treated at MSKCC using a dynamic statistical formula, (Https://www.mskcc.org/nomograms/prostate/pre-op),I have a 11% chance of organ confined disease, a 86% risk of extra capsular extension, a 37% risk of lymph node involvement, and a 31% risk of seminal vesicle invasion Based on my biopsy. The real question comes down to quality of life prognosis vs survivability.

I also found Life expectancy per SSA tables for my age for a healthy (66) year old. Life expectancy per SSA tables (https://www.ssa.gov/OACT/STATS/table4c6.html) is predicted to be 16.93 years.

( A period life table is based on the mortality experience of a population during a relatively short period of time. Here we present the 2013 period life table for the Social Security area population. For this table, the period life expectancy at a given age is the average remaining number of years expected prior to death for a person at that exact age, born on January 1, using the mortality rates for 2013 over the course of his or her remaining life.)

I do not know my quality of life expectations due to every time I look it up on the internet I get information for drugs and treatment centers and this makes it almost imposable to find true unbiased statistics please help.

If you're concerned about QOL during treatment, I can help a bit since I had similar treatment, although I also had HDR brachy. And I can anecdotally tell you about others experience with radiation in general. Most find radiation SEs to be transient and mild-to-moderate. I had some short-term urinary issues, but that was about it. Some suffer from fatigue.

HT, however, is no picnic. Totally saps libido and the hot flashes are very bothersome. I felt like it aged me 10-15 years. But...eventually you finish it and things come back to normal. I'm a few years out now and have no real SEs to speak of.

Now if you're concerned about how you'll do as a Gleason 9, there are a lot of G9 folks here that are doing quite well. (Myself included as a G9 with similar stats to you.) I'd have to say I think your chances of doing well are also good. If I were you, I would assume you'll do well until someone tells you otherwise.

Good luck with your treatment...I know it can be a slog, but eventually you'll come out of the other side of the rabbit hole!

I compiled a list of adverse effects on quality of life after primary IMRT. The typical side effects of ADT would be on top of those.

Hi Ace. Reading your details, even with the G9, I'm happy to see you have some great options available to you. I also feel your PSA being only at 7 is also a positive sign for potential of localization. ADT I believe is a necessity at this point to control it while you work out your treatment options.
Personally, I am very optimistic about your prognosis if you get busy on it now, and I hope you keep us updated regularly.

Hi Easy Ace, like Michael T, I am a G9 going through similar treatment but also with the HDR Brachy added. I agree with MT that the ADT sucks but it is just part of the journey. Unfortunately I had some adverse lower intestinal track issues during IMRT that caused daily diarrhea so my radiation treatment plan was changed to stop hitting those areas. Other than that, the radiation was painless and almost forgettable. I wish you luck on your journey. Take care. -JR

I note with interest that the pathologist is using the new(ish) ISUP grading scale of 1-5 for the combined Gleason score. 3+3=1, 3+4=2, etc. You (and I) are a 5.

Gleason 9's have a tough row to hoe. I was told by my surgeon that "you'll be fighting this for the rest of your life" when I had RP at age 69. "But I'd be alive at age 80" and with advancements coming" I might die of something else first". The bottom line is that life has forever changed and side effects are the price to pay for longer life.You learn to live with it and adjust accordingly.
