Hello,

After being on Docetaxel since April with some success. I got the results from my latest scan from earlier this week and I find out that I now have "enumerable liver and spleen mets" the largest mass being in the liver at 62.5cm.

Beyond that, I have bilateral adrenal masses, one at 5.0cm and one at 4.6cm. My last PSA only came in at 100. I have the feeling that number is far to low.

I still have bone mets that were present at my Dx along with "diffuse" activity.

So, with all this going on, I ask what options are left for me? I have failed zytiga prior to chemo.

Disappointing, to say the least. There are many options to discuss with your oncologist, among them:

- try Jevtana instead of Taxotere - it may work when Taxotere stops working. Some oncologists like to add a platin.

- After Jevtana, rechallenge with Xtandi

- embolization for the liver mass

- Provenge

- estrogen patches

- clinical trials of new hormonals (like apalutamide) and new immunotherapies (like Prostvac or Keytruda), or the DynaMo trial at MD Anderson that combines several therapies:

A Dynamic Allocation Modular Sequential Trial of Approved and Promising Therapies in Men With Metastatic Castrate Resistant Prostate Cancer (DynaMO)

Thanks for the input. I will be going to the U of M next Tuesday. I will bring up the trials and treatments mentioned. I still remain hopeful.

Having been down the road of receiving the liver met dx with Gary, it is a real slap in the face. Please keep trying, though, because there is hope. You have a lot of options still that may beat those mets back. I'm so sorry you are having to deal with the knowledge of new mets and we will be sending you positive energy and prayers through radar love.

You could have the liver and spleen mets radiated with CyberKnife and try to gain some time this way.

Is "U of M" the U of Michigan? If so, who do you see there?

I hope you get some insightful direction from your appointment. You have a lot going on.

Just wanted to reach out with a handshake of support to you --- you've always been a loyal brother of mine here on the forum --- I'm mindful of what you are going through, CADguy ---

Meanwhile, I've got a couple of close friends in this battle, and they did a second round of chemo treatments, and they are experiencing success following that --- it's a tough road to travel upon, but it definitely looks like you've got some new treatment options to explore --- including the various "chemo cocktails" that can be more specifically blended for individual patients.

Glad you are seeking out some expert advice from your medical team --- and I admire your resiliency and FIGHTING SPIRIT that is evident in your post.

We're all standing right beside you, CADguy --- lending our support to you --- you've got my unwavering loyalty in all of this, my friend.

I have a friend with liver mets, who is going strong 14 yrs after his diagnosis. You have said a lot with a great attitude when you wrote"still remain hopeful". Keep that attitude and it will go a long way. Looks like you are getting top flite treatment at U of Mich, and also good advice and support here.
Keep the faith brother, were here fighting this thing alongside you!

Thank you all for your responses. I am looking at many different option now. I will bring them up to Dr. Megan Caram At the University of Michigan Next week.
