My impression is that it is extremely good news when your PSA drops to an undetectable level. A celebration is in order! If PSA doesn't drop to undetectable levels, that's bad because it means there are still prostate cells in your body producing PSA. Then you would need to head straight into another round of treatment.
The unfortunate news is that PSA can remain undetectable for years, and then start rising again. That means, I think, that there were dormant prostate cancer cells still lurking somewhere in your body, somehow left behind by the surgery, and that they wake up and begin growing again, producing PSA.
The puzzle to me is why and how these cancer cells can remain dormant for so long, and why they re-activate themselves years later.
