Last time I looked, in the United States about 25,000 guys died each year from Prostate Cancer ..... this is not a small number.
If PSA testing is decreased this number will not go down and I will bet a bunch of money it will go up unless a viable alternative is developed first.
To attack an inexpensive screening process because docs push unneeeded and expensive treatments is ridiculous. In my mind there should be more screening and less treatment.
This guy should look at the number of treatments that have been performed on low grade PCa patients at John Hopkins ..... now that would be a number worth looking at ..... and I am sure he has access to the data.
It seems simple enough, seems being the key word. The deaths per 100K males peaked at 39 about
1991-93, and dropped steadily to 19 in 2012. about
an extra 20 per 100K(or about
twice as many) died of PC before the advent of PSA testing. Starting pretty much exactly with the increasing frequency of PSA testing.
Now, there is probably an argument that this is not due to PSA testing alone, but improved treatments. Maybe. Personally, I go with it is mostly the early detection made possible by PSA testing that is the key, especially as both started together about
the early 1990s. Too big of a coincidence.
However, many diagnosed would not die or even suffer before one of the other well known killers get to them, in fact they would never even know they had PC. But most of the 20 per 100K - probably about
twice the current #s dying from PC, would die. So about
an extra 27,000 would die if we were still dying at the 1990 rates? (again, some- how much? - of that drop in numbers is improved treatment, but I think it is mostly early detection. I could be wrong of course.)
So I guess 1 argument is: Are the lives of 27,000 men per year worth the side effects- many quite life altering- the other 200,000+ men per year(those diagnosed but who are mostly in the somewhat low risk group) will suffer?
But there is another angle: Many of those 200K men who have a PSA leading to Bx and diagnosis, do not need treatment or the SEs that go with it.
