I don't post much, but I'm an occasional window peeper on here. Just curious to find out if it makes any difference the amount of SVI involved. My urologist informed me that the amount of invasion was about
the size of a pencil lead. Hope all are having a great fourth of July holiday. Thanks.

I don't know, but maybe the less the better? Although whatever the amount, it means it is not contained, which of course would be preferable if it is contained. No one mentioned how much SV I had, only that I had it.

First phone call from the surgeon post-op he told me that both seminal vesicles were invaded, and that would require further research. I had a vague understanding of what that involved. Later in person I asked what news there was on the SVI and he just said both had been invaded.
Last week I told him I had heard there were different forms of SVI and I asked if mine was distal or direct (as that is what I heard the different types were known as) and he said he didn't know what that meant, but that both were involved.
I know some guys have only one involved and some have both or "bilateral". So I don't know how much difference it makes if at all. It's probably like being a little bit pregnant. If the SV's are involved, it just means it's not good.

My understanding is what matters is the kind of tissue invaded. If it is still in the proximal prostatic tissue, that's good, but if it has invaded the inner tissues, that's bad.

Thanks for the feedback. I wonder if distal vs direct has any correlation to proximal vs inner tissue invasion.

The doctors at Mayo told me that if the SV is involved they believe there is a high risk that margins are also involved even if the pathologist said the margins were negative. So for SV+ patients they treat more aggressively with radiation.

"SV mucosal invasion was associated with adverse outcome [HR 4.2 ], while only 15.8% of tumours with muscle wall invasion alone recurred."

Distal Pistol... blah blah blah I guess i over simplify things...but if you had surgery and its a pencil lead size and they remove the vessels which is std procedure...its not like its gushing out all over the place...yeh it could have gotten out...omg...horrors...we better do something like yesterday....the sky is falling...3 +4 ummm some AS with that.... or the the cynic might say and I will...some medicos will grab any ring that goes ca-ching... the thing that constantly amazes me is that...pre primary tx seems
to goes like this on this forum...ah man...chill pca is slow...slow...slow...take your time...lots of options...post tx...man throw the sink it at hurry...hurry...lose the <...ouch...game over.......right.... 0.02 to .03...get on it!!! an over simplification sure...by a hair...be the ball tommy be the ball...release your self...Post script
...this post in no way is meant to diminish those who suffering from advanced pca...au contraire it is meant to contrast those folks from those...so many...so many...who suffer from the "sickness unto death" Angst...anxiety...worry...feeding the bad dog...cheerleading from all involved... those alive dying...those dead living....lordy lordy...ax the knot...
