In the Katz 9 year SBRT study, the freedom from biochemical recurrence rate for low and intermediate risk groups is listed at 95% and 89% respectively. Does "favorable" intermediate (3+4) with psa <10 fall in the 95% or 89% group?

His 7-yr bRFS for favorable intermediate risk was 93.5%. It was 95% at 5 years, so not much change.
The recent 5-year SBRT trial reported a 5-yr bRFS of 97% among
all
intermediate risk patients.

Go to both places. Speak with both docs. Tour as much of the facilities as you are able. My guess is that your gut will let you know where you are most comfortable.
Katz is legendary. Zelefsky or Mychalczak at Memorial Sloan are at the top of the RO food chain as well.
You are making a decision between two options where both are great choices.

Dr Katz probably has more experience than anyone. He certainly follows up with patients longer than anyone and carefully studies them. He has added immeasurably to the knowledge about
SBRT. He found a protocol through trial and error over the years that seems to work well with very low toxicity. He has a rather ratty looking place in Flushing and is a bit of a character. I really enjoy the guy, but if gravitas and fancy digs are important, he's not your man.
Zelefsky comes from a brachy background and eventually got into SBRT. His assistant used to be Zach Zumsteg, but he left to do SBRT at Cedars Sinai in LA last year. I suppose Mychalczak that Steve mentioned replaced him. Zelefsky has been experimenting with dose and uses different linacs. I'm sure he does excellent work too.
