Scar Tissue In Head of Penis

Had da Vinci radical prostatectomy 8 weeks ago with very
Little post surgery issues with Ed or incontinance. I am 48 and had several months of doing kegels before the surgery and also my prostate was normal size. No issues before other than high PSA 5.1 and Gleason score 7 cancer. Bottom line all went well with surgery except I developed scar tissue at the tip of my penis to the point where I was basically peeing out of a pin hole. I just went back in this past surgery to do a cystoscope with dilation. Going back tomorrow to have catheter removed and I am told it is 50/50 this will work? Has anyone else had any issues similar to this?

What you experienced can happen. My BIL had a RP three years ago. I was advising him about what to expect during and after his surgery. When I talked to the BIL after he had the catheter removed he said it very painfully when the nurse removed the catheter. He said the pain was at the tip, near the opening of his penis. I said it sounds like your urethra was injured ( abraded ) as the catheter was removed. A day or two later he said there no pain from the tip area.

Then four weeks after his surgery I get a call from him. He said that his urine flow had slowed to a trickle and he was passing small pieces of plastic in his urine. Since I had many years experience with urinary strictures and recognized the symptoms I told him to call his surgeon immediately. He had a dilation the next morning and had to endure another catheter for ten days. The surgeon acknowledged that the removal of the first catheter had probably caused the stricture at the tip of the penis. Also the bits of plastic were partially dissolved sutures from the surgery. At the time of the dilation the BIL was told there was a chance that the stricture could return. Luckily in his case he has had no farther issues in the past three years. To expand a bit on what NKinney said, because I think he is probably right, what you probably had is "referred pain" from irritation of the nerves where they pass near the stitches in your urethra near your bladder. Your brain perceives pain signals coming in on a particular nerve fiber as originating at the end of the nerve but if the signal comes from some irritation along the run of the nerve you still feel it where the nerve ends.
