Updating my 'signature' and will continue to add information.

I'm between my first MRI and my first TRUS Biopsy without general sedation.

Anything better than Valium before I get the Local?

Yes, talk to your doctor, tons of good drugs out there.

You might ask for some anti-anxiety meds.

They should give you a relaxant before the procedure. I brought an ice pack and sat on it on the way home. It actually is not that bad. Just kind of well - - different and not as bad (to me at least) as having one's teeth cleaned, ha, ha!

I’m a fan of Clonazepam (Klonopin) to take the edge off the anxiety.

What is the significance of a "lesion"? Is it an indication of accelerated growth of tissue? Is it different from a tumor? Can a lesion be benign and still cause problems? Can a lesion or tumor be benign and still exit the prostate capsule?

You should ask for either a "periprostatic nerve block" or a "pelvic plexus nerve block"-- use those words. If your urologist doesn't know how to find those nerves, ask him to recommend one who does. They first start with an intra-rectal application of a 5% prilocaine/lidocaine cream to numb the injection site. When I had it done this way I was awake and alert throughout and honestly did not feel a thing. When I had it done without the nerve block, with just a local injection instead, I found it very painful and my BP plummeted. When I had it done with general anesthesia, I was loopy for a day afterwards. A nerve block is the "goldilocks" choice. (I think propofol might be good too - but you're not likely to get that - they'd have to bring in an anesthesiologist.

Also make sure you get adequate antibiotics before, during and afterwards. Let your uro know which antibiotics you've taken in the past, so he won't give you one you're resistant to.

Yes, a lesion can be benign (e.g., a cyst) or malignant. The only way to know is to biopsy.

A lesion is any anomaly that is observed, but not expected. A dentist sees dark spots on my x-rays due to infections and calls them "lesions". It is an all-purpose term for "not expected". Don't panic.

I have had five biopsies, all done at Johns Hopkins by the "best of the best". No anesthesia, just local numbing gel, and Cipro. No big deal with pain. Quick and relatively easy. So, I can't imagine why men would need being knocked out for a few short pinpricks. That sounds really like overkill.

But, afterwards is different. I had two really bad biopsy aftermaths. Pain and tripled psa for six months. Hemorrhaging seen on later MRI's. This despite taking rectal swabs before the biopsies.

But, those awful after effects could not have been foretold, nor would being knocked out have changed anything.

MRI is not a perfect technique yet, but pirads4 and capsule abuts cannot be ignored. Good luck with the biopsy.
