Here is a quick update.

After some starts and stops, I have chosen SBRT (Cyberknife) at UNC-CH.

Fiducials installed today. In 25 years someone can hunt for the gold in my ashes. Next Friday I have simulation and MRI. Treatment should begin shortly thereafter.

The Urologists today was curious about my decision process, going from Active Surveillance, to treatment option review, and finally my selection of SBRT. He also asked if I was considering hormone therapy, which I thought was a little odd given my scores. However, I told him that I discussed it with the RO, and that it was suggested but not strongly recommended.

For those considering SBRT, the fiducial placement was a piece of cake compared to my two previous biopsies. I went back to work and felt fine. I only noticed blood in my urine once, right after the procedure. I hear that the worst is now behind me.

I will post more after treatment begins for the benefit of those considering SBRT.

Great news on your decision ....sounds like all the ro guys are opting for this kind of treatment !!.....sounds like it`s all down hill from here for you ....I read very few negatives after treatment ,as I`m sure you have ....good luck 3putt !!! David

I agree that ADT would be inappropriate. Urologists are often clueless about radiation oncology. Tell me more about UNC-CH SBRT. What dose do they give? 5 treatments every other day? Which linac do they use?

It sounds like you've done your homework, 3 putt. Congratulations on not only the decision process but also on having the worst behind you (literally and figuratively). My experience was the opposite of yours, though. My biopsy was a painless non-issue, while the fiducial placement was about 15 minutes of breathing through gritted teeth cursing the doc doing the procedure. Either way, the worst is definitely behind you.

The treatment phase of SBRT is easy; mine was actually relaxing. Hope yours is, too.

3 Putt good for you. It does get easier now but get some flowmax and aleve and have it with you when you begin TX just in case some urinary issues arise.

SBRT is 'perfect' for cases like yours; very high rate of success.

Congratulations on getting past the big decision. Agree that the fiducials were pretty much a non-event. My RO required a catheter to have some CT bladder contrast and it proved to be a bit of an issue filling the bladder with the solution. However, got it all done and we are 20% on the way, four more treatments to go. Hope there are some birdie putts in your future!
