Cryo has a long history. It was the first kind of ablation therapy ever used and is covered by Medicare (and I would assume the VA). I don't know if they'd cover FOCAL cryoablation, however, which is what you'd be interested in. The results have gotten a lot better in the last few years. here's one example:

Interesting TA but the "Bulge" and "possible EPE" statements from the MRI just keep popping up in the back of my head when it comes to thinking about treatment. By the way, got lost in your blog last night and all of its clickable links.. eyes got glazed over and didn't retain much I don't think.. will try to gather myself and slow down next time.. good stuff tho.. The bulge is probably a direct result of your TZ enlargement, which tends to cause a bulge on the anterior side.

@TA. that is interesting, as I wondered how A TZ lesion would create a Bulge indicating EPE.. If I'm a possible mach for SBRT I think Stanford is in the VA "choice" program.. starting tomorrow I'm going after it in one way or another. Ed

Hi EZ,

In late 2015 I came very close to choosing RP for my TZ 4+3 with 52 gm prostate. I'd had increasing urinary difficulties for the prior 2-3 years, and I dreamed of once again being able to blast the ceramic off the urinal. What pushed me to SBRT was its better mortality nomograms. Also, my mpmri showed the tumor was abutting the edge of the prostate so I wanted the 5-10mm positive margins that my doctor recommended. Finally, I was concerned about the chance of incontinence with RP.

Now 28 months later, I'm happy with my outcome. My urinary performance is better than for many years (although no ceramic chipping) and there are no lingering side effects from the radiation. I did have a fairly big wrinkle in recovery. From months 9-18 after the treatment I had significant incontinence requiring a full pad most days. The doctor described my situation as having a radiation "flare" that can cause pain and/or incontinence for 2-12 months. So I fell right in that category with my 9 months of drama.

Wow.. Great Story and Great outcome so far.. Grats Man.. I have no bad vibes about RP but I just don't think I'm the perfect candidate for it.. Ed

I had Focal Laser Ablation on my Transition Zone tumor 5 weeks ago. Feeling no negative effects from the surgery whatsoever. In fact, I now can whiz again like I was in my 20’s. My tumor was abutting the fibromuscular stroma as well. Will have my first PSA, post surgery, in August. Although early, I have no regrets about my choice of FLA versus RALP or SBRT, the two other options I considered.

Please keep us posted on this, hoping for a great outcome for you.. as I become a little more enlightened on the subject. I realize, I might have run across a Clinical study on FLA that was still recruiting.. have to go back thru and look again. Ed

Not a very good option for you - most studies exclude men with 4 positive cores.

Ed, I had the same statements in my MRI, but when they cut it out, there were no signs of EPE, so just to let you know that a bulge on the MRI may not be anything at all.

Ed, I had the same statements in my MRI, but when they cut it out, there were no signs of EPE, so just to let you know that a bulge on the MRI may not be anything at all.
