Study: SBRT and systemic treatments for oligometastatic PCa

Hoo boy... I hesitate to post this link. Treatment of oligometastatic disease (5 or less mets), seems to go off the rails in some of our discussions. Is it worth zapping a few isolated mets? Does it really help?

Again, I'll post this link for consideration/discussion. It's a full-text link so you can read the whole study. This is a pretty small study (64 patients), and has a fairly short follow up time (median follow up 15.2 months).

Stereotactic body radiation therapy seems to be an effective treatment for metastases from PC. Prospective trials are necessary to better define selection of patients and to evaluate combination of SBRT and new systemic drugs in castration resistant patients.

There are quite a number of these. How do you know what would have happened if they didn't get metastasis directed therapy?

For some reason, Italian researchers are the biggest offenders in drawing unwarranted conclusions.

Oh, exactly. We have agreed on this in the past. I'm not sure why they still publish papers about this.

Hi Redwing....Thanks for posting this information. I am in the middle of the oligo process and have no idea how it will work out so would have to be considered on the fence as to it's effectiveness. Time will tell. Here is what I have done so far.
September 2016, PSMA PET with DCFBC tracer at National Institutes of Health in Bethesda, MD.
Results:There is an abnormal intense DCFBC uptake fusing to a left common iliac lymph node. There is a very subtle focus of DCFBC uptake fusing to a small sclerotic bone lesion in the right ischium.
Next I started a search for someone to treat both areas. Search ended in Feb. 2017 with a large teaching medical facility in New Orleans. They opted to treat only the lymph node and wait and see about the bone lesion.PSA going in was 3.5 March 2017 and dropped to 1.1 in July of 2017. Started to rise slowly in Sept. 2017. Local scans June 2018 showed questionable bone lesion in right ischium enlarging. No longer questionable. No other changes or problems found by scans. Currently August of 2018 PSA is 2.9. SBRT to ischium lesion will begin locally week of Aug. 13. No hormones during my journey yet.
Please do not allow my decisions to influence yours. Mine is a work in process and could turn out to be a complete failure. My route has been against conventional wisdom and I may well live to regret it. Make your decisions based on the best information you can get concerning YOUR case.
