Regarding medicare, I see much truth in what JNF and halbert have written. It is a very complex system and so much is typical of something "designed by committee". In other words, there are a lot of moving parts and no simple answer. Often times those who believe they can fix it with a simple solution have NO IDEA of how difficult it is to actually make a complex system work.
and as for the other topic of capitalization and punctuation consider how easy or easier it is to read a paragraph above compared to what one often sees these days where people compose with their thumbs on a smartphone and what should have been well thought out becomes just a stream of ideas that comes out as one very very long paragraph with no structure at all and personally i for one find very difficult to read especially when ppl change words into letters and u have to interpret as you read which may be easy for some but not for me okay enough ranting
BTW JNF, "indignant care" is not the same as indigent care. Though I did enjoy the chuckle.
