Can I Avoid My 1Yr Anniversary Biopsy?

I just got my latest PSA results and it came back at 2.5. All my stats are list below. I know my Urologist wants to do an MRI Biopsy on me this summer as it will be one year since my diagnosis(2 of 12 15%and 30% G6). With my OncoType DX results putting me in the very low risk category and all of my subsequent PSA being lower(this one being normal) than the original that started this journey I wondering how hard and fast the rule is for a biopsy on the 1st Anniversary. Obviously, I view fewer biopsy’s as a goal but I would also like to postpone it til earlier next year so I can reloaded my already depleted HSA. Does any of my logic make sense or am I being careless with my health?

Having an MRI targeted biopsy within a year of the original diagnosis is standard protocol in many AS programs.

Having this confirmatory biopsy has been shown to significantly reduce the chances of a more serious cancer being missed, and to assure that you are correctly placed in AS.

After these tests, your schedule may include an MRI every two years, and up to four years between biopsies, assuming that psa and other tests are stable.

I would follow the standard protocol. If done, the risk of AS is very low. I was also low risk G6 only 1 core of 54 samples and that core only had 2%, oncotype very low risk and ended up with a G7.

I second the above two comments. It's really too risky not to follow the recommendations developed by knowledgeable medical doctors.

If your gonna do AS, do it the right way. I agree with the others. To do otherwise, is a risk not worth taking.

My older brother was on AS and almost did not get the 2nd biopsy a year later, as all the other imaging and testing came back as no change, biopsy said otherwise. This was in a top NYC facility with state of the art MRI imaging..Just sayin.....

He ended up having SBRT with Zelefsky, they want to do another biopsy one or two years out after treatment, don't think he's going for that one

This why Active Surveillance is very different than Watchful Waiting. Don’t confuse the two.

Get the biopsy, follow the protocol, stay on top of this.

Remember, higher grade PCa expresses less PSA based on the cancer volume. So, you could have very slight increase in PSA and it could be due to a G7 or worse developing. That is why PSA alone is not sufficient for AS.
