Partha - look at my signature, I really wish my doctor would have sent me to the urologist when my PSA was 4.1 at age 65 based on what I now know. I agree with F8 and you should visit a urologist to begin the evaluation process. Could be nothing after another or different PSA, digital exam, and possibly MRI. Just take it slow and see what develops.
PSA is not the best indicator but it’s all we got that is simple and cheap to start with. Your PSA history and any family history of PCa would be important for the doctors to look at also.

It is at the top of the normal range for your age. Do you have a PSA history? Why did you have the test? Please provide more information. Have you had a prostate exam? What were the results?

Before I was diagnosed, my PSA had jumped from 3.2 to 4.1 in one year. My regular doctor strongly discouraged me from doing anything about it, as he said it was borderline "normal". Six months later, it was up to 6.7 and I was off to the urologist. With what I know now, I would have gone running to the urologist at 4.1. I sent a scathing handwritten letter to the regular doctor, and I never heard from him or saw him ever again.

It all depends on what the PSA trend is. You can be perfectly normal without cancer at 4.23 if it is only progressing with the age related prostate enlargement that is a normal course of aging. On the other hand if, as in Ramon and other’s cases it is increasing steadily then it could signal cancer.
From your one reading of PSA no one can tell you anything other than to investigate it. Normally that would include a DRE, which should have been performed, more frequent testing like quarterly, to detect any trend, and perhaps a PHI blood test that is helpful when PSA is under 10 to determine whether a biopsy is indicated.
It is good that you are concerned. Now do the investigation.

My psa has been rising for 7 years now, have had 3 negative mri’s and 4 negative biopsies. Go figure. Check out my signature.

Honestly: Don't know, I'd need more information to say. A single PSA test, while interesting, isn't enough. That PSA is a little on the high side for your age, but could be harmless or a sign of an enlarged prostate or a bad day at the lab.
What is your PSA history?
What I do know is that, you should ask a urologist that question and maybe get a followup PSA along with some other tests and discuss your medical history with the doc. The biggest worry you should have now is that a URO will likely want to do the famous DRE to check things out.

Welcome...as others have said, both we, and you need more information.
Can you see a urologist soon, if you haven't already? I don't know if starting an alpha blocker like silofast D8, which I believe is used to treat an enlarged prostate is the best course of action, unless that's what your diagnoses is.
Get an expert urology opinion first, do post your other medical stats, and best of luck.

Thanks a pile for your replies. I'm off to USA tomorrow where I will be there for 3 months. Shall come back and consult an urologist after undergoing another PSA test. makes me feel good at the concern shown by so many.

I would suggest to not just see a urologist- see a good urologist who sees a lot of Pc cases. There is a big difference in quality of docs in US just as it is in India !
