I can't remember what I posted where. I have had two PSA tests since RALP on Nov 7. Both <.001 which is awesome. Oncologist and my urologist are still recommending I complete ART due to the pathology report.

Still very nervous about that but every opinion I have sought out has recommended RT now versus waiting. Especially given the Intraductal designation from Epstein.

I have a radiation guy I like. His name is Dr Weed. New facility in Indy. He is also friends with my Urologist.

Waiting to start RT until I finish healing. I took the fast way down from a ten foot ladder while doing some demo work on a pergola above our deck. My wife was asleep. Neighbor kid (18) was outside so I sent him a text and said 911 not kidding. He was a trooper and went in to wake my wife and cleared the deck so the fireman and EMTs could get to me. Waited until I was 53 to get my first ambulance ride. Fractured my ankle, the T-12 vertebrae, and a lot of cuts and bruises. That was all on March 12. Doc wants my back fully healed before we start radiation. I guess it would not be good if I have a back spasm while getting cooked laying on a table. smile

That's my update. Happy for the less than zero psa and just want to get through the radiation with minimal side effects.

Maybe I miss something from your path report, but I see most guys here are not radiated unless they clearly see a psa rising trend. Adjuvant radiation is usually recommended only for high grade, G>7, and significant extracapsular extension. I do not think the IDC-P report should change that. On the other hand, I always have the opinion, not shared by everybody here, that doctors generally know what they do in each particular case. Sorry about your accident in this stressing post-surgery period.

Yeah, healing from the fall is needed. A back spasm during radiation wouldn't be good. Sorry to hear about the interductal designation--from what I understand, that's a whole 'nother ball game for treatment, and pretty rare. I can only think of a few other guys from here who have reported that.

Well, Epstein is the Man. Seems that your doctors want to get ahead of the Intraductal diagnosis. I think I learned something from the treatment plan for your situation. I would think your team would also be recommending a course of ADT to go along with the RT?

They discussed a six month hormone treatment if my psa moved otherwise they were good with just doing radiation.

I had positive margin at Apex which was where my tumor was located. Surgeon cut into the urethra muscle as far as he was comfortable to retain continence. That slice on pathology was all tumor.

I should be out of the back brace by early June. Looking to start a couple of weeks after that. Would like a few weeks in gym to get back a bit of routine before starting as well.

Thank you for the clarification.
I understand now the need of adjuvant radiation due to a potentially dangerous margin. Anyway, the excellent three and six month ultrasensitive psa results are a very good omen. Heal soon from the fractures and good luck with radiation.
