I tend to avoid discussing my own case here but somehow I am glad you asked. I apology for the digression, too. It is not far from my (fortunately light) insommia reasons anyway.
I know most in my profile is not usual. I have got this G5+3 (for some a very high risk case) in my path after a G3+3 biopsy lightly involving 2 cores of 8 samples, some of them intentionally directed to the MRI spots that was also visible with the US guide. The doctor performing the biopsy told me that was "almost impossible" that they could miss anything significant. However, after knowing the biopsy results, I still expected an upgrade because of the mpMRI report, though not this particular grading that is very rare.
I have read with detail my path report. It is not the more common 5b grade but the 5a grade, a papillary growing with "occasional" (whatever it means) comedonecrosis. Also, "less majoritarily" (I try to translate, but this is not good grammar in any language), there was an extended G3 pattern. Therefore, I guess the amounts of both pattern are not very different.
In a recent publication, Epstein and others reported that most of the revised G5+3 biopsies and path reports turned out to be something else ranging from G6 to G10. Go figure.
In any case, I was not advised to do anything else since I also got a path T2 stage.
about the DRE nodule, this is also a strange story. The urologist following my PSA decided to make a DRE just before the MRI. Negative. I had another one just before the biopsy. Negative. But a month later, after diagnostic and in my first to the surgeon his assistant performed that last DRE and it was positive on the side of the MRI lesion and the greater charge in the biopsy. Better sensitivity or just influence of the tests?
Finally, I will comment my PSA readings after surgery. I was terrified by the first value 0.08. Later, it was down and became fluctuating. This is the ordinary PSA test but they report it up to the second digit. I have some clues that it may be one of those sometimes confounding Siemens tests. From other people taking tests in the same place I know they are always in the range of most of my results and also that they never see a < sign. I asked my surgeon how many people of his patients get a zero there and his amazing answer was "nobody". Again, go figure.
I have followed topics here about revising biopsies, taking genomic tests, insisting in ultrasensitive PSA, etc. Some of these actions are more difficult for me and it would take me too much energy figuring out how to proceed. But, also, I have the policy of not going beyond what I am advised, unless it is clearly against to what I know. That is, I just want to know what I need to know about what to do next. All these revisions may add something on prognosis but not about currently needed action. I have read through many stories, here and in other places like Yananow, in addition of a lot of technical publications. Therefore I consider myself I reasonably well informed patient. And, as I claimed above, I would not terribly happy to have an extensive discussion on the pros and cons about my particular case.
But I thank you are interested, especially because your questions did not include any particular advice. I notice we are on similar boats and I hope you remain permanently with low PSA readings meaning remission.

I was never good sleeper. Most of my adult life ( college and after) between 5 and 6 hours with quite often less then 5 hours. This was never problem for me. I basically finished college studying at night and working all day long, very often more then 10 .
During my post prostatectomy time, my sleep was bigly disrupted with toilet times, I couldn’t fall asleep or I would wake up hour or two after falling asleep and that was end of night for me. I did not want to take any meds beside 1 herbal supplement.
It took me about 30 months post RP to get back to my normal sleep pattern.

My prostate was 90g before my TURP, which reduced it to 30g. By the time of my RP for PCa, it had grown back to 64g. If I had had a crystal ball predicting the cancer, I would have gleefully done a RP instead of the TURP! I was experimenting with sildenafil for my BPH, and now find erections no problem with its use and even possible without at times. One plus about RP for high-grade PCa is that we men with prostate-confined cases can often avoid adjuvant therapy (and hopefully salvage as well).

I have concluded that anxiety about the future of my cancer is pointless. I imagine myself failing at an advanced age of this or that, and thinking back on years of needless worry. On the other hand, if I do encounter BCR in x years, so be it -- I'll just deal with it then.

If I'm lucky I sleep from 9pm to 3am ..... If I'm not lucky I fall asleep earlier and get up earlier. I recently found this:

I don't have a problem getting up at 3pm.... I'm out the door by 4am for my morning walk....which is the ONLY times that works for me.

Then there's the part about my CAT that wakes me up at 3..... Am not sure if I'd sleep later but the darn cat has an unbeatable schedule. (luckily she old and won't be around much longer)

I can't decide if it's good news or bad....but I generally take to naps.... One at 7am'ish and another after lunch.

I’ve been struggling since going on ADT. Historically I’ve not had a problem falling asleep until now. Staying asleep is a different matter. I’ve been getting very creative with Rx’s and combined with a nightcap but even that isn’t working now. I understand Eligard May be the cause and just had my 2nd shot two weeks ago. Good times. I have a Dr appt tomorrow and will inquire about Zoplicone. My Dr had me try Ramelteon but it’s like holy water...nada... no bueno...I took Ambien for years but it was making me stupid. I checked with the pharmacist today and Xtandi lessens the effects of Ambien as well as many other sleep Rx’s. I feel like I need a sledgehammer and all that’s available is a fly swatter, hence the cocktail. No sleep is not good. Gotta find a solution.

I had sleep apnea, got a tonsillectomy in 1997, stopped that completely. Quit caffeine after breakfast when it started interfering with my sleep.
