wewillie, welcome to our club that none of us chose to join. It's tough, I get it. There is also a LOT of hope. I'd second Djin's suggestion to send your slides to Epstein at Johns Hopkins. His word is the final word. about a third of the time the second opinion will come back with a downgrade. That being said, you have 8/12 positive cores. That's a lot. And, you've got a lot of G4 hanging around.
Your scans will tell you a lot about what your long term might be like.
What I'd suggest you do is a few additional things:
1. Let us know where you are. What city are you in or near? With that info, we might suggest where to go for top care. If you're not sure, it's a good start to google "NCCN centers of excellence". This is a listing of the top cancer facilities across the US>
2. Plan on making consultation appointments with specialists in all the various subspecialties for treatment. With as much G4 as you're showing, you're probably not a candidate for Active Surveillance.
3. Try to relax. It's not the end of the world. You have every chance of getting treated and cured. Again, you owe it to yourself to talk to all the treatment experts you can. Remember, your urologist is a surgeon, he will recommend surgery and may try to steer you that way and discourage you from talking to the radiation experts. There are good and bad points to all treatments. IF your scans come back showing the cancer has escaped your prostate, then radiation is likely a better course. Surgery can't get it all under that condition.
